【摘要】 目的评价保留前列腺包膜及射精功能的腹腔镜膀胱全切回肠新膀胱术的临床价值。方法回顾性分析2008年7月至2010年9月在我院行保留前列腺包膜及射精功能的腹腔镜膀胱全切回肠新膀胱术的男性患者临床资料,共24例,年龄35~72岁,其中19例术前有经尿道膀胱肿瘤电切术史。临床病理分期:T2N0M015例,T3aN0M06例,T3bN0M03例。结果本组24例患者均成功施行手术,术中出血量200~700ml,平均360ml。手术时间平均380min。随访7个月~2年,3例出现轻度尿失禁,其余尿控好,11例能完成性交及射精功能,远处转移3例,其中已死亡2例。结论保留前列腺包膜及射精功能的腹腔镜膀胱全切回肠新膀胱术根据我们的早期随访结果疗效满意,明显提高了患者的生活质量。更多还原

【Abstract】 Objective To evaluate the safety and efficiency of laparoscopic radical cystectomy with ileal neobladder: retaining the prostate capsule and ejaculatory function. Methods The clinical data of 24 patients who underwent laparoscopic radical cystectomy with ileal neobladder: retaining the prostate capsule and ejaculatory function from July 2008 to September 2010 were analyzed retrospectively. Of the 24 patients 19 had underwent transurethral resection of bladder tumors. The patients aged from 35 to 72 years. Clinical stages were T 2 N 0 M 0 15 cases, T 3a N 0 M 0 6 cases, T 3b N 0 M 0 3 cases. Results 24 patients were operated successfully. The mean bloss was 360 ml (200-700), The mean operative time was 380 minutes. After 7-24 months follow-up, mild urinary incontinence in 3 cases, sexual intercourse and ejaculation in 11 patients, distant metastasis in 3 cases and death in 2 cases were observed. Conclusion Based on our early follow-up, the procedure of laparoscopic radical cystectomy with ileal neobladder: retaining the prostate capsule and ejaculatory function was safe and effective and that can improve the patient’s quality of life.更多还原
