术中实时超声辅助神经外科手术

【摘要】 目的评价术中实时超声在神经外科手术中的应用价值。方法本组手术病例45例,包括脑病变切除术30例,血肿清除术5例,内镜颅内异物取出术2例,开颅活检术2例,内镜透明隔囊肿开窗术3例,内镜三脑室底造口术3例。所有手术均在术中实时超声辅助下进行。结果所有病变均在超声辅助下精确定位,并完整切除。神经内镜可在导航下精确进入脑室。本组无症状恶化及死亡者,未发现手术并发症。结论术中超声导航可确定病灶和脑室的位置,决定皮层的切口,动态了解病灶情况,判断病变的切除程度,避免损伤重要血管。

【Abstract】 Objective To evaluate the clinical value of intraoperative ultrasound guided neurosurgery. Methods The intraoperative sonography-guided neurosurgery was performed in 45 patients:30 resection of intracranial lesions,5 heamatoma 、 clearing,2 extirpation of intracranial foreign-body,2 craniotomy for biopsy,3 endoscopic septostomy,3 endoscopic third ventriculostomy (ETV). Results The intraoperative sonography can accurately locate the intracranial lesions , and it is helpful to the total resection of the lesions. Neuroendoscope was inserted into ventricle under ultrasound control. No patients were deteriorated and dead. No complications occurred. Conclusion Introperative ultrasound can accurately locate the lesions and ventricle, to determine the optimum cortical openings, to know the dynamic information of the lesions resection,to judge the degree of excision,and to avoid injury of important blood vessel.
