流行性腮腺炎减毒活疫苗免疫学效果和保护效果的系统评价

【摘要】 目的通过对流行性腮腺炎减毒活疫苗(Mumps Attenuated Live Vaccine,MuV)免疫学效果和保护效果的系统评价,为完善MuV免疫策略提供依据。方法采用系统评价方法,以疫苗、流行性腮腺炎、效果为检索词,全面检索中国知识资源总库期刊全文数据库(1994~2011年)及其世纪期刊(1979~1993年)和万方学术期刊数据库(1982~2011年),对纳入文献进行系统分析。结果符合纳入标准的文献23篇,16篇涉及MuV免疫学效果研究,3篇为MuV免疫持久性的研究,4篇为病例对照研究。对MuV免疫学效果和保护效果系统评价显示:血凝抑制抗体阳转率为23.21%~93.18%,IgG抗体阳转率为83.2%~97.7%;3年后抗体阳性率保持>75%;Meta分析显示,疫苗的保护效果为85%。结论接种MuV后,IgG抗体阳转率较高,并能保持较好的持久性和保护作用。

【Abstract】 Objective To review systematica immunology effect and protective efficacy of mumps attenuated live vaccine and to provide the basis for improving immunization strategy of mumps.Methods The literatures on immunology effect and protective efficacy of mumps live vaccine were searched in China national knowledge internet(1979-2011)database and Wanfang database(1982-2011).Systematic review and meta-analysis were performed to analysis the selected literatures.Results 23 papers were enrolled.16 papers were studies on immunology efficacy of mumps attenuated live vaccine,3 papers involved the immune persistent of mumps attenuated live vaccine and 4 papers were case-control studies about protective efficacy of mumps attenuated live vaccine.The results of systematic review showed that the hemagglutination inhibition antibody positive conversion rate was 23.21%-93.18%,the IgG antibody positive conversion rate was 83.2%-97.7% and positive rate after 3 years remained above 75%.A meta-analysis showed that protective efficacy of mumps vaccine was 85%.Conclusion Mumps attenuated live vaccine can stimulate high-titer IgG antibodies and maintained long-term protection against mumps.
