声辐射力脉冲成像技术检测肝占位性病变的初步研究
Initial Study of Acoustic Radiation Force Impulse for Differentiation of Hepatic Benign and Malignant Tumors
【摘要】 目的探讨声辐射力脉冲成像技术(ARFI)对肝占位性病变鉴别诊断的临床价值。方法应用ARFI技术对71例(共88个病灶)肝占位性病变患者进行检查,对检查结果进行对比分析,鉴别诊断其良、恶性。结果原发性肝癌组、肝转移癌组与肝良性病变组剪切波速比较差异均有显著性意义(P<0.05),原发性肝癌组与肝转移癌组剪切波速比较差异无统计学意义(P>0.05)。以2m/s作为恶性病变的诊断标准,ARFI成像鉴别肝脏良、恶性肿瘤的敏感性为92%,特异性为86%。结论 ARFI技术可以直观地显示肝肿瘤的硬度,有效评价肿瘤组织的弹性变化,为肝脏良、恶性病变的早期鉴别诊断提供更多依据。
【Abstract】 Objective To investigate the clinical value of Acoustic Radiation Force Impulse(ARFI) technology for differentiation of hepatic benign and malignant tumors.Methods In total,71 patients with 88 focal hepatic lesions were used to examine with ARFI.The results were compared and analysed to distinguish benign and malignant tumors of liver.Results There was significant difference of share wave velocity between malignant group and benign group (P<0.05) but no significant difference between primary hepatic malignant tumors and metastatic liver tumors(P> 0.05).With a cut-off value of 2 m/s for share wave velocity,the positive predictive value and specificity for malignancy were 92%and 86%respectively.Conclusions ARFI imaging could provided additional qualitative information regarding the stiffness of tumors for differentiation of hepatic benign and malignant tumors.
