围着床期和HCG日子宫内膜3DU-PDA参数评价子宫内膜容受性
Assessment of Endometrial Receptivity by Three-dimensional Ultrasonography and Power Doppler Angiography during Peri-implantation Period and HCG Administration Day
【摘要】 目的探讨IVF前一自然周期围着床期及绒毛膜促性腺激素注射日(HCG日)子宫内膜三维超声能量多普勒血管成像(three-dimensional ultrasonography and power Doppler angiography,3DU-PDA)参数评价子宫内膜容受性并预测体外受精胚胎移植(IVF-ET)结局的价值。方法选择南方医院生殖医学中心2009年4月至2009年12月行IVF-ET治疗的67例妇女为研究对象,追踪助孕结局分为临床妊娠组(31例)和未妊娠组(36例)。比较两组一般情况、临床资料和围着床期及HCG日子宫内膜3DU-PDA参数。结果临床妊娠组和未妊娠组之间围着床期子宫内膜3DU~PDA参数没有显著性差异(P均>0.05)。临床妊娠组HCG日子宫内膜下FI高于未妊娠组,差异有统计学意义(t=2.398,P=0.019),但预测妊娠的ROC曲线下面积仅为0.475。结论在辅助生殖技术中测量HCG日子宫内膜3DU-PDA参数有利于评价子宫内膜容受性,但是特异性不高。
【Abstract】 Objective To evaluate the role of endometrial parameters by 3DU-PDA during peri-implantation period and HCG administration day in the prediction of endometrial receptivity.Methods 67 patients undergoing IVF/ ICSI-ET treatment from 1st April 2009 to 31st December 2009 were analyzed retrospectively.Endometrial tissue was obtained by disposable suction tubes from uterine cavity after 7 days of ovulation.Endometrial tissue was used to Scanning electron microscope observation pinopods.All subjects were divided into two groups according to their IVF-ET outcome:clinical pregnancy(31 cases);nonpregnant group(36 cases).Data analysis was carried out on the patients’characteristics, clinical information and 3DU-PDA parameters.Results The subendometrial FI on HCG day for the clinical pregnancy group was significantly higher than that of the nonpregnant group(P = 0.019).The ROC curve analysis showed that subendometrial FI on the HCG day had the area under the curve<0.5(0.475).Conclusions In the IVF/ICSI-ET cycles,3DU-PDA parameters on HCG day is helpful to assess endometrial receptivity,but the Specificity is low.
