定量超声弹性成像技术与X线钼靶对乳腺小结节诊断价值的比较

【摘要】 目的评价定量超声弹性成像技术与X线钼靶对乳腺良恶性小结节的诊断价值。方法对53例女性患者共72个乳腺实性小结节病灶用定量超声弹性成像技术测定病灶应变率与周围正常乳腺组织弹性应变率比值,判断病灶良恶性,并与手术病理结果及术前X线钼靶检查结果进行对照。通过与术后病理结果比较,绘制ROC曲线图,获得区分乳腺小结节良恶性应变率比值的临界值。结果 (1)乳腺良性组应变率比值为2.46±1.11,恶性组应变率比值为5.85±1.69,二者比较差异有统计学意义(P<0.05)。(2)通过ROC曲线确定良、恶性肿块弹性应变率比值的临界点为4.38,其特异性、敏感性、准确性分别为87.09%、90.24%和88.89%,X线钼靶的特异性、敏感性、准确性分别为77.74%、73.17%、70.83%。统计学结果显示二者比较差异有统计学意义(P<0.05);且两者联合运用可进一步提高乳腺小结节病变性质诊断的准确性(P<0.05)。结论定量超声弹性成像技术对乳腺小结节病变性质的诊断价值明显高于X线钼靶,而联合检查能进一步提高恶性病灶的检出率。

【Abstract】 Objective To evaluate the diagnostic value of quantitative ultrasound elasticity imaging and molybdenum X-ray photography(MX-rP)on the screening and diagnosis of small breast carcinoma.Methods A total of 53 female patients with 72 solid breast nodules were detected with quantitative ultrasound elasticity imaging system.The elastic strain rate ratio of tumor and the surrounding normal breast tissue were calculated to determine whether the lesion was benign or malignant.In addition,the results were compared with the results of pathology and molybdenum X-ray photography.The results were also compared with pathological results to obtain the critical value of the ratio of strain rate by drawing ROC curve for the diagnosis of benign or malignant breast nodules.Results①The strain rate ratio of benign and malignant leisions was 2.46±1.11 and 5.85±1.69 respectively.There was significant difference between them(P<0.05).②The critical point was calculated with ROC curve.When the critical point was 4.38,the sensitivity,specificity and accuracy for real-time strain imaging system were 87.09%,90.24%,and 88.89%.The sensitivity, specificity and accuracy for X-ray were 77.74%,73.17%,70.83%.There was significant difference between them(P<0.05).Conclusions The diagnostic value of real-time strain imaging system is significant higher than MX-rP. Combination of real-time strain imaging system and MX-rP can be more valuable than real-time strain imaging system or MX-rP alone in the screening and diagnosis of small breast leisions.
