原发性高血压肾动脉阻力指数与早期靶器官损害的关系

【摘要】 目的研究肾动脉阻力指数(RRI)与原发性高血压早期靶器官损害相关临床指标的相关性,评价其在高血压早期靶器官损害中的诊断价值。方法原发性高血压患者74例,其中RRI≥0.7组29例,RRI<0.7组45例,正常对照组32例。应用超声检测RRI、颈总动脉内膜-中层厚度(CIMT)和左室舒张功能指标(Ve/Va)。同时检测肌酐等生化指标及尿微量白蛋白。结果高血压组RRI值明显高于对照组(P<0.01)。RRI高组中患者年龄、收缩压、脉压差、CIMT、血肌酐和尿微量白蛋白较高(P<0.05),Ve/Va和舒张压较低(P<0.05)。相关分析显示,RRI同年龄、收缩压、脉压差、CIMT和血肌酐呈正相关(P<0.05),同Ve/Va和舒张压呈负相关(P<0.05)。多元线性回归分析显示,年龄、脉压差和CIMT同RRI独立相关。结论 RRI同心血管形态学和血流动力学改变相关,可作为高血压早期靶器官损害的一个重要检测指标。

【Abstract】 Objective To investigate the association between renal resistive index(RRI) and early target organ damage in primary hypertensive patients,and evaluate the clinical significance of RRI in diagnosing target organ damage in primary hypertension.Methods 76 subjects with hypertension and 32 control subjects were enrolled.According to RRI,the hypertensive patients were divided into two groups:45 patients with RRI less than 0.7 and 29 with RRI more than 0.7.RRI,carotid intima-media thickness(CIMT) and left ventricular diastolic function parameter(Ve/Va ) were measured by ultrasonography,and determined by biochemical indicators and microalbuminuria.Results The RRI in hypertensive patients was significantly greater than that in controls(P<0.01).Patients in RRI high group showed an increased age,systolic blood pressure,pulse pressure,CIMT,serum creatinine and microalbuminuria(P<0.05),but a decreased diastolic blood pressure and Ve/Va(P<0.05 ).Relevant analysis revealed positive correlation with RRI for age,systolic blood pressure,pulse pressure,CIMT and serum creatinine,and negative correlation for diastolic blood pressure and Ve/Va.Multiple linear regression analysis indicated that age,pulse pressure and CIMT could influence RRI independently.Conclusions RRI is related to the change of cardiovascular morphology and hemodynamics,and may be a useful indicator for diagnosing early target damage in hypertensive patients.
