CD99和MMP-2在妊娠滋养细胞疾病中的表达及意义
Expressions and significances of CD99 and MMP-2 in gestational trophoblastic disease
【摘要】 目的:研究黏附因子CD99及基质金属蛋白酶-2(MMP-2)在水泡状胎块和水肿性流产中的定位和表达及二者表达的相关性。方法:采用免疫组化SP法检测30例水肿性流产(HA)、20例完全性水泡状胎块(CHM)、20例部分性水泡状胎块(PHM)患者中CD99和MMP-2的表达水平。结果:①CD99主要表达于细胞滋养细胞胞膜,在HA、CHM和PHM中的表达率分别为49.83%、25.85%和25.40%;②MMP-2主要表达于合体滋养细胞胞质,在HA、CHM和PHM中的阳性表达率分别为36.7%、80.0%和85.0%;③CD99在HA组织中的表达率高于CHM和PHM组织,MMP-2在HA组织中的表达率低于CHM和PHM组织,且差异均有统计学意义(P<0.05);经Spearman相关性检验,CD99和MMP-2在三者中的表达率均呈负相关〔γ(HA)=-0.133,P<0.05;γ(CHM)=-0.154,P<0.05;γ(PHM)=-0.153,P<0.05〕。结论:CD99在水泡状胎块(HM)中表达下降,可能与其影响滋养细胞黏附及分化有关,MMP-2在HM中表达上升,可能与CD99表达下调,从而激活MMP-2表达,导致MMP-2过表达有关。二者均参与了HM的发生和发展,且CD99表达的下降在一定程度上促进了MMP-2的表达,从而为HM的早期诊断和临床治疗提供了依据。
【Abstract】 Objective:To research the locations and expressions of adhesion factor CD99 and matrix metalloproteinase-2(MMP-2) in hydatidiform mole(HM) and hydropic abortus(HA) and their correlation.Methods:Immunohistochemical SP method was used to detect the expression levels of CD99 and MMP-2 in 30 cases with HA,20 cases with complete hydatidiform mole(CHM) and 20 cases with partial hydatidiform mole(PHM).Results:①CD99 mainly expressed in the cell membrane of cytotrophoblast,the expression rates of CD99 in HA,CHM and PHM were 49.83%,25.85% and 25.40%,respectively.②MMP-2 mainly expressed in the cytoplasm of syncytiotrophoblast,the expression rates of MMP-2 in HA,CHM and PHM were 36.7%,80.0% and 85.0%,respectively.③The expression rate of CD99 in HA was significantly higher than those in CHM and PHM,and the expression rate of MMP-2 in HA was significantly lower than those in CHM and PHM(P<0.05);the result of Spearman correlation test showed there was a negative correlation between CD99 expression and MMP-2 expression in HA,CHM and PHM(γ(HA)=-0.133,P<0.05;γ(CHM)=-0.154,P<0.05;γ(PHM)=-0.153,P<0.05).Conclusion:The decrease of CD99 expression in HM may be related to its impact on adhesion and differentiation of trophoblast;the increase of MMP-2 expression in HM may be related to down-regulation of CD99 expression,activation of MMP-2 expression and over-expression of MMP-2.Both CD99 and MMP-2 participate in the occurrence and development of HM,and the decrease of CD99 expression promotes the expression of MMP-2 to a certain extent,and it provides a reference for early diagnosis and clinical treatment of HM.
