布地奈德吸入治疗呼吸道合胞病毒或鼻病毒感染所致婴幼儿第一次喘息的疗效分析
Analysis on the curative effect of budesonide inhalation in treatment of first infantile wheezing induced by respiratory syncytial virus infection or rhinovirus infection
【摘要】 目的:探讨布地奈德雾化吸入治疗呼吸道合胞病毒、鼻病毒感染所致婴幼儿喘息第一次发作时的疗效。方法:2004年5月~2006年5月因第一次轻中度喘息发作在苏州大学附属儿童医院哮喘中心住院治疗的患儿212例,入院后随机分成治疗组与对照组,对照组采用空气压缩泵吸入万托林雾化液;治疗组在吸入万托林雾化液的基础上吸入布地奈德悬液。采用直接免疫法检测新鲜痰液标本中的呼吸道合胞病毒(RSV)、腺病毒(ADV)、流感病毒(IFV)和副流感病毒(PIV);用RT-PCR法检测标本中的鼻病毒(RV)基因。采用UniCAP荧光酶系统进行过敏原过筛试验。收集患儿的出生史及特异性疾病的家族史。比较治疗组与对照组中单纯感染RSV或RV患儿的平均住院时间;同时比较单纯RSV感染患儿与单纯RV感染患儿的平均住院时间;比较治疗组中RV感染患儿与对照组中RV感染患儿的平均住院时间;比较治疗组中RSV感染患儿与对照组中RSV感染患儿的平均住院时间。结果:①治疗组单纯感染RSV或RV患儿平均住院时间为(6.71±1.79)天,对照组单纯感染RSV或RV患儿平均住院时间为(7.25±1.64)天,两组相比差异无统计学意义(P>0.05)。②RSV感染患儿平均住院时间为(7.25±1.86)天,RV感染患儿平均住院时间为(6.64±1.54)天,两组相比差异无统计学意义(P>0.05)。③治疗组RV患儿平均住院时间为(6.01±1.03)天,与对照组RV患儿平均住院时间〔(7.5±1.73)天〕相比,差异有统计学意义(P<0.05)。④治疗组RSV患儿平均住院时间为(7.46±2.13)天,与对照组RSV患儿平均住院时间〔(7.06±1.61)天〕相比,差异无统计学意义(P>0.05)。结论:布地奈德吸入治疗能缩短RV喘息患儿的平均住院时间,但对RSV喘息患儿的住院时间无影响。
【Abstract】 Objective:To explore the curative effect of budesonide inhalation in treatment of first infantile wheezing induced by respiratory syncytial virus(RSV) infection or rhinovirus(RV) infection. Methods:212 infants with the first mild-moderate onset of wheezing hospitalized in asthma center of children hospital affiliated to Suzhou university from May 2004 to May 2006,then they were divided into treatment group and control group,the infants in control group were treated with inhalation of ventolin atomized liquid by a air pressure pump;the infants in treatment group were treated with budesonide inhalation on the basis of inhalation of ventolin atomized liquid.Direct immunofluorescent method was used to detect RSV,adenovirus(ADV),influenza virus(IFV) and parainfluenza virus(PIV) in fresh sputum samples.RT-PCR was used to detect RV gene in the samples.Allergen screening test was conducted with Unicap100 luciferase system.The birth history and family history of atopic diseases of the infants were collected.The average hospitalization times of simple RSV-infected infants or simple RV-infected infants in treatment group and control group were compared;at the same time,the average hospitalization times of simple RSV-infected infants and simple RV-infected infants were compared;the average hospitalization times of simple RV-infected infants in treatment group and control group were compared;the average hospitalization times of simple RSV-infected infants in treatment group and control group were compared. Results:The average hospitalization times of simple RSV-infected infants or simple RV-infected infants in treatment group and control group were(6.71±1.79) days and(7.25±1.64) days,respectively,there was no significant difference between the two groups(P>0.05).The average hospitalization times of simple RSV-infected infants and simple RV-infected infants were(7.25±1.86) days and(6.64±1.54) days,respectively,there was no significant difference between the two groups(P>0.05).The average hospitalization times of simple RV-infected infants in treatment group and control group were(6.01±1.03) days and(7.5±1.73) days,respectively,there was significant difference between the two groups(P<0.05).The average hospitalization times of simple RSV-infected infants in treatment group and control group were(7.46±2.13) days and(7.06±1.61) days,respectively,there was no significant difference between the two groups(P>0.05). Conclusion:Budesonide inhalation can shorten the average hospitalization time of infants with wheezing induced by RV infection,but it has no impact on the hospitalization time of infants with wheezing induced by RSV infection.
