【摘要】 目的:探讨月经过多诊刮后置曼月乐的疗效价值。方法:2008年6月～2009年10月在两家医疗机构选择有排卵型月经过多患者90例,接受3种治疗方式,对所有病例治疗后进行为期1年的随访,记录其治疗效果。结果:共71例有随访结果,其中接受诊断性刮宫同时置入曼月乐(A组)、月经第4～7天直接置入曼月乐(B组)、单纯诊断性刮宫(C组)3种方式的患者分别为24、22、25例。PBAC评分在第1、2、3、6、12月3组差异均有统计学意义(P<0.05);血红蛋白在第1、2、3、6月差异无统计学意义,12月则显示差异有统计学意义(P<0.05)。点滴状出血发生率在第1、2、3、6月3组间差异均有统计学意义,B组的发生率一直最高,A组次之,C组发生率最低。结论:曼月乐治疗有排卵型月经过多可有效减少月经量,提高血红蛋白且患者满意度较好。结合诊断性刮宫可准确诊断导致月经过多的器质性病变,值得在临床推广。更多还原

【Abstract】 Objective:To explore the efficacy and value of Mirena placement in cases with menorrhagia after dilatation and curettage.Methods:90 patients with ovulatory menorrhagia were selected from two medical institutions from June 2008 to October 2009,then they received three therapeutic modes,all the patients were followed up for one year after treatment,and the therapeutic efficacy was recorded.Results:71 patients had follow-up results,including 24 patients in group A(receiving Mirena placement and diagnostic curettage at the same time),22 patients in group B(receiving Mirena placement at 4～7 days during menstrual cycle) and 25 patients in group C(receiving simple diagnostic curettage).There was significant difference in PBAC score at 1,2,3,6 and 12 months among the three groups(P<0.05);there was no significant difference in hemoglobin content at 1,2,3 and 6 months among the three groups,but there was significant difference in hemoglobin content at 12 months among the three groups(P<0.05).There was significant difference in incidence of spotting bleeding at 1,2,3 and 6 months among the three groups,the incidence of spotting bleeding in group B was the highest,followed by group A,the incidence of spotting bleeding in group C was the lowest.Conclusion:Mirena can reduce the volume of menstrual blood,increase hemoglobin content with a high satisfaction in treatment of patients with ovulatory menorrhagia.Mirena combined with diagnostic curettage can diagnose the organic lesions inducing menorrhagia accurately,which is worthy to be popularized.更多还原
