【摘要】 目的通过研究肾病综合征患儿凝血和纤溶相关指标的变化,探讨肾病综合征发病机制,为肾病综合征的病情判断和治疗提供依据。方法检测63例肾病综合征患儿的血浆纤维蛋白原(FIB),抗凝血酶(AT-Ⅲ),D-二聚体(D-D)和纤维蛋白降解产物(FDP)的水平,同时检测25例肾病综合征患儿尿液FDP和尿ALB的水平,分别与健康对照组进行比较,同时分析肾病综合征组患儿血浆AT-Ⅲ与血清ALB、尿液FDP和尿液ALB的相关关系。结果肾病综合征组血浆FIB、D-D、FDP的水平明显高于正常对照组,血浆AT-Ⅲ水平明显低于正常对照组,尿液FDP的水平明显高于正常对照组,差异均有统计学意义(P<0.01)。肾病综合征组血浆AT-Ⅲ与血清ALB呈正相关,结果有统计学意义(r=0.573,P<0.01);尿液FDP与尿ALB无相关关系,结果无统计学意义(r=0.330,P=0.107)。结论肾病综合征患儿体内存在凝血和纤溶亢进,并有肾脏内局部凝血和纤溶异常,观察这些指标的变化有利于监测疾病的发展和治疗。更多还原

【Abstract】 Objective To investigate the pathological mechanism of nephrotic syndrome by study on changes of coagulative and fibrinolytic system in children with nephrotic syndrome,and to provide a reliable evidence for the clinical diagnosis and treatment of nephrotic syndrome.Methods The levels of fibrinogen(FIB),anti-thrombin(AT-Ⅲ),D-dimer(D-D),fibrin degradation product(FDP) in blood plasma were determined in 63 nephrotic syndrome cases;The levels of FDP and ALB in urine were determined in 25 nephrotic syndrome cases;They were compared with those in the control group;The relationships between AT-Ⅲ and ALB,FDP and ALB were analyzed by correlation analysis.Results The levels of FIB,D-D,FDP in blood plasma in the nephritic syndrome group were significantly higher than those in the control group;The level of AT-Ⅲ was significantly lower than that of the control group(P<0.01);The level of AT-Ⅲ in blood plasma was positively correlated with ALB in the serum(r=0.573,P<0.01),and FDP in urine was not correlated with the level of ALB in urine(r=0.330,P=0.107).Conclusion There are some abnormalities in coagulative and fibrinolytic system in children with nephrotic syndrome.Studies on the changes of coagulative and fibrinolytic system in children with nephrotic syndrome may be beneficial for monitoring the progress and treatment of this disease.更多还原
