【摘要】 目的分析和探讨前列腺间叶性肿瘤的临床病理特征。方法对14例前列腺间叶性肿瘤的临床病理特征进行形态学观察、免疫组化染色以及术后随访。结果 10年中共行前列腺穿刺活检1072例,诊断前列腺间叶性肿瘤14例(1.3%),其中原发性间叶性肿瘤12例(86%),包括前列腺特殊间质肿瘤4例(1例恶性潜能不确定的间质肿瘤,3例间质肉瘤)、孤立性纤维性肿瘤3例、胃肠间质瘤1例,梭形细胞肉瘤(倾向为间质肉瘤)4例;继发性间叶性肿瘤2例(14%)。临床表现为进行性排尿困难12例,所有患者经直肠指诊均可触及前列腺明显增大;12例血清PSA 0.06～3.87 ng/ml,平均1.67 ng/ml。结论前列腺间叶性肿瘤主要为原发性间叶性肿瘤,前列腺特殊间质来源肿瘤是前列腺间叶性肿瘤中最常见的肿瘤类型,其次为孤立性纤维性肿瘤,患者往往较年轻。更多还原

【Abstract】 Objective To study the clinicopathologic features of stromal tumors of prostate.Methods Fourteen cases of prostatic stromal tumors were analyzed through clinical data statistics,light microscopy,immunohistochemistry and follow-up.Results From July 2000 to January 2010,a total of 1072 patients were performed by prostate core-needle biopsy and 14 cases of prostatic stromal tumors were diagnosed.Detection rate of prostatic stromal tumors was 1.3%(14/1072) over the 10-year period.Of the 14 prostatic stromal tumors,12 cases were primary tumor accounting for 86%(12/14) and secondary tumor was accounting for 14%(2/14).Patients ranged in age from 20 to 70 years(mean 42.4 years).The major presenting symptom was urinary obstruction.All prostates of 14 cases were significantly enlargement,and the serum PSA values of 12 cases ranged from 0.06 to 3.87 ng/ml(mean 1.67ng/ml).In 12 cases of prostate primary stromal tumors,there were 4 cases of prostatic specialized stromal tumors(including 1 case of STUMP and 3 cases of stromal sarcoma),3 cases of solitary fibrous tumor and 1 case of GIST,and the other 4 cases were spindle cell sarcoma.There were 2 cases of secondary prostatic stromal tumors,in which 1 case of peritoneal malignant mesothelioma(sarcomatoid variant) and the other one was PNET arising from abdominal cavity.Conclusions Prostatic stromal tumors dominantly are primary stromal tumors of prostate and that prostatic speciliazed stromal tumor is the most common type,and solitary fibrous tumor is the second common type of the prostatic stromal tumors.Most of the patients are young adult.更多还原
