【摘要】 目的为提高鼻部外伤并颅颌外伤患者临床救治质量,多学科协作模式下进行综合治疗。方法对65例鼻部外伤并颅颌外伤患者按GCS评定颅脑外伤患者伤情,入院后经多学科协作模式下进行综合治疗以及相关预后进行分析。结果 65例鼻部损伤合并颅颌外伤患者中,鼻骨骨折40例,鼻中隔软骨脱位及鼻中隔骨折8例,合并颌骨骨折28例,眶骨骨折3例,颅脑损伤情况:GCS>8分41例,占63.1%,GCS≤8分24例,占36.9%。入院后立即施行清创缝合术57例,开颅手术15例,同期鼻骨骨折鼻中隔骨折脱位复位术2例、颌骨骨折切开复位术3例、采取保守治疗50例。病情稳定后施行颅脑手术21例,施行鼻骨骨折复位术33例,鼻中隔成形术5例,颌骨骨折切开复位内固定术20例,眶骨骨折复位1例。对本组所有病例进行观察,患者均生命体征稳定,刀口感染2例,视力丧失1例,听力下降1例,鼻部术后鼻腔无粘连,外形良好,颌骨骨折复位固定术后咬功能恢复良好者22例,出现咬功能恢复不良1例。结论对患者住院期间的多学科交叉医疗管理模式下建立起来的多种治疗方式已成为有效治疗鼻部外伤合并颅颌外伤的综合策略之一。更多还原

【Abstract】 Objective To improve the nasal trauma and craniofacial trauma patients clinical treatment quality,multidisciplinary collaboration mode to carry out comprehensive treatment.Methods 65 cases of nasal trauma and craniofacial trauma patients by GCS assessment in patients with craniocerebral trauma injury,after admission to the multidisciplinary collaboration mode of comprehensive treatment and prognostic analysis.Results 65 cases of nasal trauma and craniofacial trauma patients,40 cases of nasal bone fracture,dislocation of septal cartilage of nose and nasal septum fracture in 8 cases,28 cases of jaw fractures,3 cases of orbital bone fracture,brain injury: GCS>8 in 41 cases,63.1%,GCS<8 in 24 cases,36.9%.After admission to immediate debridement and suture in 57 cases,craniotomy operation in 15 cases,In the nasal bone fracture nose separates the bone fracture dislocation reduction 2 examples same time,open reduction of fractured jaw in 3 cases,conservative treatment in 50 cases.After the stable condition,Purposes of 21 cases of craniocerebral operation,implementation of nasal bone fracture reduction in 33 cases PLASTY of nasal septum in 5 cases,open reduction and internal fixation of 20 cases,1 cases of orbital bone fracture reduction.All the cases were observed in the group,all patients with stable vital signs,incision infection in 2 cases,loss of vision in 1 cases,hearing loss in 1 cases of postoperative nasal cavity,nasal no adhesion,good shape,jaw fracture reduction and fixation of posterior occlusal function,22 cases had a good recovery,1 cases appeared occlusal function recovery bad.Conclusion The patients during hospitalization and multidisciplinary medical management mode set up a variety of treatment modalities have become effective in the treatment of nasal trauma associated with craniofacial trauma integrated strategy.更多还原
