鲍曼不动杆菌医院感染分布及耐药性探讨
Analysis of the hospital infection and drug resistance of acinetobacter baumannii
【摘要】 目的:探讨鲍曼不菌动杆菌(Aba)医院感染分布及耐药性,为临床合理应用抗菌药物提供参考。方法:采用回顾性方法统计分析我院2010年分离的Aba临床分布情况及对其耐药性进行分析。结果:全年共分离出菌株1 690株,Aba 282株,位居第2位,但在革兰阴性杆菌中占第1位;282株Aba主要分离自重症监护病房(ICU),占44.3%,其次是烧伤科、呼吸内科和神经外科等住院患者的标本;以痰液标本为主,占78%;多黏菌素E的敏感率最高,为99.27%,其次是亚胺培南和美罗培南,但耐药率也高达68.68%和68.57%,对其他抗菌药物的耐药率均>76%。结论:Aba分离率高,多重耐药情况严重,治疗上建议联合用药,临床医生应重视合理使用抗菌药物,加强消毒隔离和无菌操作,防止Aba的播散和流行。
【Abstract】 Objective: To investigate the clinical distribution and drug resistance of Aba and offer the guidance for clinical application of antibiotics reasonably.Methods: The clinical distribution and drug resistance of Aba collected from our hospital in 2010 were retrospectively studied.Results: 1 690 strains of bacteria were isolated the whole year totally.Among them 282 strains were Aba and it holded second place.But Aba was in first place among the Gram-negative bacteria;282 strains of Aba were isolated from ICU(44.3%).The next were the samples isolated from the patients in departments of the Burn,the Respiratory Diseases and the Neurosurgery.Sputum specimens were the main samples(78%).The sensitive rates for polymyxin E were the highest(99.27%).And the next were imipenem and meropenem,but the drug-resistance rate of them were 68.68% and 68.57%.The drug-resistance rate of Aba for the other antibiotic was over 76%.Conclusion: Aba has a high infection rate clinically and the multiple-antibiotic-resistant situation is serious.Combined drug treatment is better to use clinically.The doctor should apply antibiotic reasonably,tighten disinfection isolation and aseptic technique,so as to prevent the spreading and prevalence of Aba.
