【摘要】 目的:探讨颈椎前路减压植骨固定治疗下颈椎骨折脱位的疗效。方法:选取2005年1月～2010年2月于我院进行治疗的62例下颈椎骨折脱位患者为研究对象,将其随机分为A组(颈椎前路减压植骨固定组)和B组(颈椎后路减压植骨固定组),每组各31例。将两组患者的手术时间、出血量、并发症发生率及手术前后JOA评分、寰齿前间隙、Cobb角、生存质量评分进行统计及比较。结果:A组手术时间[(85.67±16.22)min]短于B组[(120.34±15.68)min],出血量[(125.86±32.75)ml]少于B组[(258.69±33.12)ml],并发症总发生率(6.45%)低于B组(12.90%),JOA评分[(24.1±2.1)分]及生存质量评分[生理、心理、环境及社会关系的评分分别为(76.85±7.15)、(78.59±7.69)、(79.57±8.60)、(76.68±7.66)分]均高于B组[(65.93±7.17)、(70.35±8.82)、(69.80±7.92)、(62.30±7.71)分],A组寰齿前间隙、Cobb角[(2.38±0.39)mm,(3.67±0.85)°]均小于B组[(3.89±0.49)mm,(6.20±1.28)°],两组比较,差异均有统计学意义(P<0.05或P<0.01)。结论:颈椎前路减压植骨固定治疗下颈椎骨折脱位的疗效好,优点多,可显著改善患者的生存质量。更多还原

【Abstract】 Objective: To study the effects of anterior cervical decompression bone grafting fixation in the treatment of lower cervical fractures and dislocations.Methods: 62 cases of patients with lower cervical fractures and dislocations in our hospital from January 2005 to February 2010 were selected as research objects,and they were randomly divided into group A(anterior cervical decompression bone grafting fixation group,31 cases) and group B(cervical posterior decompression bone grafting fixation group,31 cases),then the operation time,blood loss,incidence of complications and JOA score,atlantodental anterior space,Cobb angle,quality of life score before and after the treatment were analyzed and compared.Results:The operation time of group A [(85.67±16.22) min] was shorter than that of group B [(120.34±15.68) min],blood loss [(125.86±32.75) ml] was less than that of group B[(258.69±33.12) ml],incidence of complications(6.45%) was lower than that of group B(12.90%),JOA score [(24.1±2.1) point] and quality of life score [the score of physiological,psychology,environment and social relations were(76.85±7.15),(78.59±7.69),(79.57±8.60),(76.68±7.66) scores] were all higher than group B [(65.93±7.17),(70.35±8.82),(69.80±7.92),(62.30±7.71) scores],atlantodental anterior space [(2.38±0.39) mm] and Cobb angle [(3.67±0.85)°] were smaller than those of group B [(3.89±0.49) mm,(6.20±1.28)°],there were significant differences in the two groups(P<0.05 or P<0.01).Conclusion: The effect of anterior cervical decompression bone grafting fixation in the treatement of lower cervical fractures and dislocations is better,advantages is more,and it can significantly improve the quality of life.更多还原
