联合检测抗磷脂抗体、血β-HCG、血钙、平均动脉压预测妊高征的临床价值

【摘要】 目的:检测孕中期孕妇血清抗心磷脂抗体、血钙与血β-HCG的水平,探讨联合监测孕妇平均动脉压对妊娠高血压综合征(妊高征)发生的预测价值,寻求一种简便、实用、易行的预测方法。方法:对180例妊娠16~20周的孕妇进行前瞻性研究,测定其血抗心磷脂抗体(ACA)、血钙水平及血β-HCG值,同时监测平均动脉压,并观察发展为程度不同妊高征时上述指标的变化规律。结果:妊高征孕妇在孕中期血β-HCG值、ACA阳性率、平均动脉压≥85 mmHg的发生率均较正常组明显升高,差异有统计学意义(P<0.05),血钙明显下降(P<0.05)。联合四项预测指标,阳性预测值为80.95%,阴性预测值为98.74%,敏感度为89.47%,特异度为97.52%。结论:妊娠中期联合测定血ACA、β-HCG、血钙及平均动脉压可预测妊娠高血压综合征的发生,且优于任一单项预测指标。

【Abstract】 Objective:To investigate the predicting value of pregnancy induced hypertension(PIH) by measuring the level of blood anticardiolipin antibodies(ACA),β-HCG in blood,blood calcium,and the mean arterial blood pressure(mABP),to find a simple,easy and clinical value of forecasting methods.Methods:The prospective studies was about 180 cases of pregnant women(16-20 gestational weeks) whose blood were drew to measure ACA,β-HCG,blood calcium and mean mABP,and the above-mentioned indexes in various degree PIH patients were observed.Results:Blood β-HCG levels,A-CA-positive,the occurrence of MAP ≥85 mm Hg were significantly higher in pregnancies who later developed PIH than those in normal pregnancies(P<0.05),blood calcium levels were significantly lower(P<0.05).Predictor of the four indexs combined,the positive predictive value was 80.95%,negative predictive value was 98.74%,sensitivity was 89.47% and specificity of 97.52%.Conclusion:The combined detection of mid-pregnancy blood ACA,blood β-HCG,blood calcium,mABP can forecast PIH,and it is better than any single forecasting index.
