【摘要】 目的总结病房床旁中心静脉置管的经验,为临床工作提供借鉴。方法回顾性分析2007年3月至2010年8月我院556例非手术患者实施中心静脉导管置入的临床资料。结果 556例全部穿刺置管成功。86例困难置管中有41例术前评估伴有特殊情况,其中借助血管超声患者12例,将穿刺针弯成弧形进行穿刺成功者9例。有1例发生颈部血肿,为严重并发症。结论该研究总结了中心静脉置管过程中常出现的问题和解决方法。病情特殊的患者要有针对性措施,选择非常规方式实施中心静脉穿刺置管术也常常能成功。而借助血管超声技术,可避免严重并发症,提高穿刺成功率。更多还原

【Abstract】 Objective To summarize our expiences in central venous catheterization at bedside in adults.Methods Data of 556 adult patients undergoing central venous catheterization at bedside from March 2007 to August 2010 in our hospital were retrospectively analysed.Results Central venous catheterizations were achieved in all patients successfully.Eight-six patients were defined as difficult catheterization,of which,41 were accompanied by some elements with regard to difficult cannulation.Among those,12 cannulations were done by using ultrasound,9 by bending the puncture needles like an arch.Jugular haematoma occurred in one patient,which was considered as serious complication.Conclusion In this article,the most often occurred problems and methods to solve them were summarized.Actually,various techniques exist in various circumstances to achieve successful cannulation.Vascular ultrasound technology helps to avoid serious complications,may enhance one-time success rate of puncture.更多还原
