【摘要】 目的探讨乳腺癌各序列及动态增强的信号特征。方法经病理证实的45例乳腺癌,共计60个病灶。平扫采用横断面T2加权像(T2WI)快速自旋回波脂肪抑制序列及自旋回波T1WI,动态增强扫描采用快速小角度激发扰相梯度回波序列加脂肪抑制序列。观察病变平扫信号特征、增强后病灶形态学特征、早期强化率、强化曲线类型。结果乳腺癌平扫T2WI信号以等信号及高信号为主(50/60),瘤周可出现水肿(45/60),动态增强扫描以毛刺状不规则肿块居多(45/60),环形向心性强化为其主要强化方式(48/60),大多早期强化率超过100%(44/60),强化曲线以廓清型最多(37/60)。结论 T2WI瘤周出现水肿对乳腺癌有很好的提示作用,联合病灶的形态学特征及动态增强曲线可以显著提高乳腺癌的诊断正确率和多源性乳癌的检出率。更多还原

【Abstract】 Objective To explore the signal characteristics of various sequences and dynamic enhanced MRI for breast carcinoma.Methods T2 weighted image(T2WI)with fast spin echo and fat suppression,T1 weighted image(T1WI)with spin echo,and dynamic enhanced MR image of breast carcinoma in 45 cases(60 lesions)verified by histopathology were reviewed.Scan signal characteristics,enhanced lesion morphology,early enhancement rate,and enhancement curve types were observed.Results 50 lesions(50/60)of breast carcinoma showed isointensity or/and hyperintensity in T2WI.Peritumoral edema occurred in 45 lesions(45/60).Dynamic enhanced MR findings of 60 lesions include: irregular and spiculate shape in 45 lesions(45/60),centripetal enhanced fashion in 48 lesions(48/60),more than 100% prophase reinforcement rate occurred in 44 lesions(44/60),the characteristic with fast clearance of time signal curve was manifested in 37 lesions(37/60).Conclusion Peritumoral edema,combined with manifest of morphology and the type of time signal curve of dynamic enhanced MRI,may significantly elevate diagnosis precision of breast carcinoma and increase detection rate of multifocal breast cancer.更多还原
