磁共振新技术在乳腺癌诊断及其与预后因子相关性方面的研究进展

【摘要】 磁共振动态增强检查(DCE-MRI)及弥散加权成像技术(DWI)已逐渐被应用于乳腺癌的诊断,其诊断准确度、特异度均较高。Her-2、Ki-67是乳腺癌的预后因子,与部分乳腺癌组织学类型、淋巴结转移及预后有关。探索DCE-MRI、DWI与Her-2、Ki-67之间的相关性,可能对判断部分乳腺癌组织学类型,指导手术中淋巴结清扫等方面具有一定作用。部分学者认为两者间不具有相关性;部分研究证实ADC值与p53有一定负相关性;Ki-67与边缘环形强化呈正相关;Ki-67与流出型曲线正相关;内部坏死与Her-2阳性表达呈显著正相关。

【Abstract】 Dynamic contrast-enhanced MRI(DCE-MRI) and diffusion-weighted imaging(DWI) have been used for the diagnosis of breast cancer recently,with an apparently high rate of diagnosis.Her-2 and Ki-67 are prognostic factors of breast cancer that have been confirmed to be correlated with histological type,lymph node metastases,and prognosis of breast cancer.The further investigations into the association of DCE-MRI and DWI with Her-2 and Ki-67 may well be able to differentiate histological type of breast cancer and instruct intraoperative lymph node dissection.Some researchers propose that they are not associated whilst ADC value is negatively correlated with p53,Ki-67 positively correlated with ring-like enhanced margin and outflow-type curve.and interior necrosis positively with Her-2 expression.
