【摘要】 目的探讨Castleman病的CT表现,提高对其认识。方法回顾性分析经手术证实17例为Castleman病的临床资料与CT表现。结果病变位于胸部11例,腹膜后5例,腹膜后及盆腔同时发生1例。临床分型为局灶型11例,多中心型6例。病理诊断透明血管型12例,浆细胞型4例,混合型1例。CT平扫3例伴有钙化,其余病灶为等密度;增强扫描透明血管型病灶均有明显持续性强化,中间可有点状或条状低密度;浆细胞型呈轻度～中度强化。结论 Castleman病CT表现与临床类型、病理分型密切相关,透明血管型病变的CT表现具有特征性,浆细胞型无特征性影像学表现,确诊需结合组织病理学。更多还原

【Abstract】 Objective To study CT diagnostic features of Castleman disease.Methods The clinical data and CT scans of 17 patienls with histologically proven Castleman disease were analyzed retrospectively.Results The lesions were solitary (n=11) or multicentric(n=6),located in the chest(n=11),retroperitoneum(n=5),or retroperitoneum and pelvis(n=1).The histopathologic classification included hyaline-vascular(n= 12),plasma cell(n=4) and mixed(n=1) types.Apart from 3 cases with intra-tumoral calcifications,the remaining 17 lesions showed homogeneous attenuation on unenhanced CT.The hyaline- vascular type of lesions showed marked and sustained contrast enhancement with central dot or stripe hypodensity whereas the plasma cell type demonstrated mild to moderate enhancement.Conclusion CT features of Castleman disease are closely correlated with pathological and clinical types.The hyaline-vascular type of lesions has some characteristic CT features whereas the CT appearance of the plasma cell type is non-specific.Pathologic examination is required for diagnosis.更多还原
