【摘要】 目的研究腕关节早期类风湿性关节炎(RA)的磁共振成像(MRI)诊断价值。方法回顾性总结临床确诊早期RA 99例,对照组(包括骨关节炎、强直性脊柱炎、干燥综合征、银屑病、痛风、未分化结缔组织病)共85例,分析病变早期低场强四肢磁共振(E-MRI)特征。结果 RA组腕关节MRI阳性表现:滑膜增生血管翳形成(93.9%),骨与软骨受累(74.7%),伸肌腱腱鞘炎(61.6%),屈肌腱腱鞘炎(58.6%)等,以滑膜增生和骨质改变以及腱鞘炎并存诊断早期RA,其敏感性(67.6%)、特异性(96.7%)、正确率(79.8%)。结论 E-MRI对早期RA诊断、疗效随访及预后判断具有重要价值。更多还原

【Abstract】 Objective To study the appearances of extremity magnetic resonance imaging of the wrist joint rheumatoid arthritis(RA) in early stage.Methods The patients of 99 cases with RA in wrist joint who had been diagnosed clinically and radiologically were analyzed retrospectively,Another 35 cases,The patient(with OA 、AS、PSS、PSA、GOUT、UCTD)were taken as control,early appearances of E-MRI of RA in wrist joint were summarized.Results The positive findings of MRI in wrist joint as follow,synovitis and enhanced pannus(93.9%),cartilage and bone erosion(74.7%),tenosynotitis of the extensor tendons(61.6%),tenosynotitis of the flexor tendons(58.6%).Change of synovium,bone erosion and tenosynotitis were taken as diagnostic standard,the sensitivity,specificity,accuracy,67.6%、96.7%、79.8%.Conclusion E-MRI has important value in the diagnosis of early stage of the wrist RA.Early synovitis and erosion are valuable factors in the assessment of activity and prognosis of RA.更多还原
