原发性甲状腺功能减退与糖代谢紊乱37例观察

【摘要】 目的探讨原发性甲状腺功能减退与糖代谢的关系。方法采用葡萄糖氧化酶氧电极法测定血糖,采用化学发光免疫分析法测定FT3、FT4、TSH与TPOAb、TGAb。结果 37例甲减患者中DM占16.22%,IGT占5.41%,IFG占16.22%,IGR占21.62%。结论原发性甲状腺功能减退合并高血糖代谢紊乱的机理目前尚不清楚,可能与免疫、遗传及环境等因素有关,是否还与甲状腺素替代治疗有关,有待进一步研究。但其在中老年人群中发病率较高,故应引起临床医师特别是内分泌医师的重视。

【Abstract】 Objective To investigate the relationship between primary hypothyroidism and glucose metabolism.Methods Plasma glucose was determined by glucose redox enzyme assay,free thyronine(FT3),free thyroxine(FT4),thyroid-stimutating hormone(TSH) and TG-antibody,TPO-antibody were determined by ICMA(immunochemiluminometric assays).Results Of thirty-seven primary hypothyroid patients,16.22% were auompamed with diabetes mellitus,5.41% were auompamed with impaired glucose tolerance,16.22% were suffered from impaired fasting glucose,21.62% were suffered from impaired glucose regulation.Conclusion The mechanism of primary hypothyroidism auompamed with the disorder of hyperglycemia is orobably related to immunity and genetics,but the incidene is higher in the elderly population,it should be given more attention by climcian,expecially by endocrinology physician.
