非共面野在脑胶质瘤调强放射治疗中的剂量学优势

【摘要】 目的:探讨非共面射野在脑胶质瘤调强放疗中的应用特点及剂量学优势,为非共面野应用于脑胶质瘤调强放疗临床应用提供理论依据。材料方法:对6例脑胶质瘤患者分别设计5野共面调强计划和在此之上增加一个非共面野的6野调强计划,利用剂量体积直方图评价两种计划方法的正常组织照射剂量、适形度指数以及不均匀指数。结果:6野非共面计划比5野共面计划有更好适形性和均匀性;同时减少了眼球、晶体、视神经照射剂量。结论:在脑胶质瘤的调强放疗中,加入非共面野计划比共面计划有更明显的剂量学优势。

【Abstract】 Objective: To investigate characteristics and dosimetric advantage of the non-coplanar radiation field in IMRT for gliomas and to provide the theoretic basis for the application of the non-coplanar radiation field in IMRT for gliomas clinically.Materials and Methods: Six patients with brain glioma were enrolled in this study.The 5-field coplanar IMRT plan and 6-field non-coplanar plan based on coplanar plan were performed for each patient.The conformity index,heterogeneity index and dose distributions of normal tissues were analyzed with dose-volume histogram(DVH).Results: The CI and HI of non-coplanar plan were superior to the coplnanar plan.The non-coplannar plan decreased the irradiation dose of eye ball,lens and optic never.Conclusions: Obvious dosimetric advantage could be found by adding a non-coplanar field in IMRT plan for gliomas.
