多排螺旋CT三维重建在肺栓塞中的诊断价值
Multislice spiral CT pulmonary angiography in the clinical diagnosis of pulmonary embolism value
【摘要】 目的:探讨16层CT肺动脉成像在诊断肺动脉栓塞中的临床应用价值。方法:回顾分析23例已确诊为肺栓塞的患者临床与影像资料,对CTPA检查进行5mm、2.5mm、1.25mm、0.625mm薄层重建,包括容积再现(VR)、多平面重建(MPR)、最大密度投影(MIP)。结果:23例病人以胸痛、呼吸困难及咯血发生率最高,CTPA均检出栓塞灶,其中右肺5级以上动脉栓塞灶69个,左肺5级以上动脉栓塞灶59,病变累及肺动脉主干3个,左右肺动脉及叶动脉61个,栓塞率37.9%,肺段及肺亚段动脉64个,栓塞率17.9%,1.25mm与0.625mm组病灶检出率相近似,1.25mm与2.5mm组及5mm组栓塞率有差异,其中在2~3级肺动脉栓塞检出率中,x2=10.0328,P<0.05,差异有统计学意义。在4~5级肺动脉栓塞检出率中,x2=6.0644,P<0.05,差异也有统计学意义。结论:16层以上的螺旋CT能够准确显示肺动脉栓塞的部位、累及范围,可作为诊断肺动脉栓塞最主要的检查方法和确诊依据;1.25mm薄层MIP图像后处理技术是检出肺动脉栓子的最佳技术。
【Abstract】 Objective:To evaluate the 16-slice spiral CT pulmonary angiography in the diagnosis of pulmonary embolism in clinical application.Methods:Retrospective analysis of 23 cases have been diagnosed with pulmonary embolism in patients with clinical and imaging information on the CTPA examination to 5mm,2.5mm,1.25mm,0.625mm thin reconstruction,including volume rendering (VR),multiplanar reconstruction (MPR),maximum intensity projection (MIP).Results:23 patients with chest pain,breathing difficulty and the highest incidence of hemoptysis,CTPA embolization foci were detected,of which more than 5 right lung artery embolization foci 69,left lung artery embolization for more than 5 lesions and 59 lesions involved the pulmonary trunk 3,the pulse of the left pulmonary and leaves fixed 61,embolism rate of 37.9%,sub-section of lung and pulmonary artery segment 64, embolism rate of 17.9%,1.25mm and 0.625mm group detection rate of lesions was similar to,1.25mm and 2.5mm group and 5mm group thrombosis rate were different.Which in the 2~3 level in the detection rate of pulmonary embolism,x2=10.0328 P<0.05,The diffirent has statistics significance.Which in the 4~5 level in the detection rate of pulmonary embolism,x2=6.0644 P<0.05,The diffirent has statistics significance.Conclusion:The 16-slice spiral CT can more accurately display the location of pulmonary embolism,involving the scope of diagnosis of pulmonary embolism as the main basis of the inspection methods and diagnosis;1.25mm thin MIP image processing technique is the detection of pulmonary emboli in the most good technology.
