【摘要】 目的检测慢性乙型肝炎(CHB)患者不同免疫状态下外周血CD4+、CD8+T细胞表面程序性死亡受体1(PD-1)表达,探讨其与血清HBV DNA载量、ALT之间的关系。方法收集CHB患者免疫耐受期(A组,24例)、免疫活化期(B组,64例)、HBeAg阴性(C组,23例)和11例健康对照者(D组)外周血,采用流式细胞术检测CD4+、CD8+T细胞表面PD-1表达,以实时荧光定量PCR检测血清HBV DNA,同时检测ALT。结果 B、C组CD4+、CD8+细胞表面PD-1表达显著高于A、D组(P<0.05)。C组CD8+T细胞表面PD-1表达显著高于B组(P<0.05)。相关性分析发现,不同免疫状态下CHB患者CD4+、CD8+T细胞表面PD-1表达水平与HBV DNA载量及ALT均无相关性。结论 CHB患者外周血T细胞表面PD-1表达与机体免疫状态有关。肝脏炎症损伤是影响PD-1表达的重要因素,HBV DNA载量不是主要影响原因。更多还原

【Abstract】 Objective To detect the programmed death type-1(PD-1) expression on CD4+ and CD8+ T cells in peripheral blood of chronic hepatitis B(CHB) patients with different immune status and investigate its relationship with HBV DNA loads and ALT levels.Methods Blood samples were colleted from CHB patients in the immune-tolerance phase(group A,24 cases),in the immune-reactive phase(group B,64 cases),with HBeAg-negative(group C,23 cases) and 11 healthy people(group D) for detecting the expression levels of PD-1 on total CD4+and CD8+ T cells by flow cytometry.HBV DNA loads were determined by real-time fluorescent quantitative-PCR and ALT levels were examined as well.Results PD-1 expressions on CD4+ and CD8+ T cells were higher in groups of B and C than those in groups of A and D(P<0.05).PD-1 expression on CD8+ T cells was higher in group C than that in group B(P<0.05).Correlation analysis showed that expression levels of PD-1 on CD4+ and CD8+ T cells were not correlated with HBV DNA loads or serum ALT levels.Conclusion The expression of PD-1 on peripheral blood T cells is closely related to the immune status of CHB patients.The liver inflammation,but not serum HBV DNA loads,is the important factor to influence the expression levels of PD-1 on T cells.更多还原
