【摘要】 目的探讨腹腔镜直肠癌根治术对肿瘤细胞血行播散的影响。方法采用实时荧光定量RT-PCR技术检测30例开腹组与28例腹腔镜组中低位直肠癌患者术前1 d及术后1 d外周血CK20 mRNA表达指数。结果开腹组术后CK20 mRNA阳性表达率明显高于术前[76.4%(13/17)vs.56.7%(17/30)](P<0.05),CK20 mRNA中位表达指数亦明显高于术前[213.6 vs.109.7](P<0.05)。腹腔镜组术后CK20 mRNA阳性表达率和中位表达指数略高于术前[64.7%(11/17)vs.60.7%(17/28)和(193.5 vs.157.6)](P>0.05)。结论腹腔镜手术和开腹手术对直肠癌肿瘤细胞血行播散有一定程度影响,但腹腔镜手术与开腹手术相比并不增加肿瘤细胞血行播散的几率。更多还原

【Abstract】 Objective To study the influence of laparoscopic procedure on blood spread of tumor cells in patients with mid-low rectal cancer.Methods Real time fluorescence quantitative-PCR was used to detect cytokeratin 20(CK20) mRNA expression and CK20 mRNA expression index in the patients undergoing rectal cancer surgery under laparoscopic(group A) or open procedure(group B) one day before and one day after operation.Results CK20 mRNA expression rate and CK20 mRNA expression index in group B were significantly higher after operation than those before [76.4%(13/17)vs.56.7%(17/30) and(213.6 vs.109.7)](P<0.05)),which in group A were not significantly different [64.7%(11/17) vs.60.7%(17/28) and(193.5 vs.157.6)](P>0.05).Conclusion Laparoscopic procedure has less influence on the blood spread of tumor cells than open procedure in the patients undergoing rectal cancer surgery.更多还原
