肝癌患者D-二聚体、纤溶酶原、蛋白C检测的临床意义

【摘要】 目的检测肝癌患者血浆D-二聚体、纤溶酶原(PLG)、蛋白C(PC)水平变化,探讨它们在肝癌病人病情进展、疗效观察及预后的临床意义。方法以免疫比浊和发色底物法检测22例正常对照、43例肝癌患者血浆中D-二聚体含量、PLG、PC活性水平在治疗前、缓解后、病情进展时的水平变化。结果肝癌治疗前血浆D-二聚体升高、PLG降低、PC水平降低,与正常对照组有显著差异(P值均<0.01);缓解组D-二聚体明显低于初发组,PLG、PC高于初发组。结论肝癌患者存在不同程度的高凝状态和纤溶亢进,血浆D-二聚体含量、PLG活性水平对病情和预后的评估有一定的参考价值,血浆PC水平降低是反映病情危重的指标。

【Abstract】 Objective To detect plasma D-dimer,plasminogen,proteinum C levels in patients with hepatocellular carcinoma(HCC),to investigate the clinical significance of them in progression,effect observation and prognosis.Methods 22 normal controls and 43 patients with HCC were selected into the study.The plasma PLG and PC activity were measured by using chromogenic substrate assays,D-dimer was quantitatively determinated by immunoturbidimetric method.Results Compared with normal control group,the plasma PLG and PC activity were reduced significantly and the plasma D-dimer levels increased significantly before treatment in HCC(P <0.01).In remission group,the plasma PLG and PC activity increased significantly and D-dimer levels decreased significantly compared with those before treatment(P <0.01).Conclusion These results suggest that HCC patients have high condensation state and fibrinolytic hyperthyroidism of different degree,the plasma D-dimer levels,PLG activity levels have certain reference value for the evaluation of disease and prognosis,the reduction of plasma PC levels is the indicator of reflecting the severity of disease.
