【摘要】 目的探讨腹腔镜联合光动力治疗晚期胰腺癌的临床疗效。方法 39例晚期胰腺癌患者随机分为两组,治疗组19例采用腹腔镜联合光动力治疗,对照组20例为常规治疗,包括化疗,放疗,内置架植入和开腹姑息手术等。结果治疗组死亡率为15.8%(3/19),对照组死亡率为25.0%(5/20)。两组死亡率相比无明显统计学差异(χ2=0.507,P>0.05)。两组治疗前后白细胞计数无明显差异(P>0.05);两组治疗后第7天TBIL水平的变化较治疗前显著性降低(P<0.05)。治疗后第7天复查肿瘤标记物发现,两组CA199和CA125水平较治疗前显著降低,治疗组CA199水平显著低于对照组(P<0.05)。结论腹腔镜联合光动力治疗晚期胰腺癌安全有效。更多还原

【Abstract】 Objective To investigate the effect of the laparoscopy combined with photodynamic therapy in treatment of patients with advanced-stage pancreatic carcinoma.Methods 39 cases of advanced-stage pancreatic carcinoma were divided into the treatment group and the control group randomly.19 cases in the treatment group were treated with the laparoscopy combined with photodynamic therapy.Meanwhile,20 cases in the control group were treated by other palliative therapies: chemotherapy,radiotherapy,stent implantation,and open abdominal surgery,etc.Results The mortality in the treatment group was 15.8%(3/19),which was lower than 25.0%(5/20) of the control group with no statistical significant difference(χ2 = 0.507,P >0.05).The levels of WBC in both groups were not found any change(P >0.05).However,the levels of TBIL,CA199,and CA125 in both groups started to significantly reduce at the 7th day after treatments,comparing to pre-treatments(P <0.05).Moreover,the level of CA199 in the treatment group was lower significantly than that of the control group(P <0.05).Conclusion The combination of laparoscopic surgery and photodynamic therapy is safe and effective for patients with advanced-stage pancreatic carcinoma.更多还原
