【摘要】 目的:评价PTBD内外引流对胰腺的影响。方法:通过对110例梗阻性黄疸行PTBD内外引流和胆总管下段经壶腹放置支架,观察患者术前术后血淀粉酶的变化,64例高位胆管梗阻行内外引流和46例低位胆管梗阻支架治疗,比较两者胰腺炎的发生率。结果:AMY升高占44.5%(49/110),35例AMY大于3倍正常值,引流组胰腺炎的发生率明显大于支架组(P<0.001)。结论:胰腺损伤和胰腺炎是PTBD较常见的并发症,胰腺损伤与引流管或支架是否通过壶腹有关。更多还原

【Abstract】 Objective:To evaluate the response of pancreas in patients with biliary obstruction after PTBD.Methods:110 consecutive patients with unresectable biliary obstruction underwent PTBD treatments,PTBD internal-external grainage in 64 cases and inserting stent across ampullae in 46.Serum amylase(AMY) levels were measured before and after PTBD.The effects of pancreas were compared with drainage group and stending group.Results:AMY increased in 49 cases(44.9%).AMY was three times of normal baseline levels in 35 cases.The number of the patients with hyperamylasemia or panreatitis after internal-external grainage procedures was more than patients who were inserted stents(P<0.001).Conclusion:The damnification of the pancreas is hackneyed complication of PTBD.Pancreatic response is related with grainage catheter and stent across ampullae.更多还原
