新生儿窒息患儿血浆组织因子和组织因子途径抑制物水平变化
Changes of plasma tissue factor and tissue factor pathway inhibitor in newborn with asphyxia
【摘要】 目的探讨新生儿窒息血浆组织因子(TF)和组织因子途径抑制物(TFPI)含量变化及其意义。方法选择64例新生儿窒息患儿和同期32例正常足月新生儿(对照组)作为研究对象,采用酶联免疫吸附法(ELISA)分别测定脐血、窒息极期和窒息恢复期血浆TF、TFPI含量。结果新生儿窒息时,脐血TF含量明显升高(P<0.05),TFPI含量略有升高,TFPI/TF比值变小,与对照组比较二者差别无统计学意义(P>0.05)。窒息极期,外周血TF含量进一步升高,TFPI含量则出现下降,TFPI/TF比值继续变小,三者与对照组比较差别有统计学意义(P<0.01);重度窒息组TF含量比轻度窒息组高(P>0.05)。窒息恢复期,外周血TF含量明显回落,轻度窒息组与对照组比较差别无统计学意义(P>0.05),但重度窒息组仍然高于对照组(P<0.05);TF含量轻度窒息组比重度窒息组低,但差别无统计学意义(P>0.05);TFPI含量3组之间比较差别无统计学意义(P>0.05);但TFPI/TF比值窒息组同对照组比较仍然明显降低(P<0.05);窒息恢复期TFPI/TF比值轻度窒息组比重度窒息组高,差别无统计学意义(P>0.05)。结论新生儿窒息时,TF和TFPI含量出现明显改变,TF持续升高,而TFPI含量初期升高,极期降低,TFPI/TF比值降低,其变化程度与窒息程度呈正相关关系。
【Abstract】 Objective To study the changes of plasma tissue factor(TF) and tissue factor pathway inhibitor(TFPI) in newborn with asphyxia.Methods Sixty four newborn with asphyxia and thirty two normal newborn(control group)were selected.The levels of TF and TFPI in plasma of umbilical venous,asphyxiation extreme period and convalescent period were determined by using enzyme-linked immunosorbent assay(ELISA).Results Compared with control group,the plasma levels of TF in umbilical venous in newborn with asphyxia rose significantly(P<0.05),while TFPI rose slightly(P>0.05),TFPI/TF decreased a little(P>0.05).In extreme period,TF levels of the two asphyxiated groups went on increased significantly,but TFPI and TF/TFPI decreased,they were both significantly compared with the control group(P<0.01).The TF levels of severe asphyxia group was higher than that of slight asphyxia group(P>0.05).In convalescent period,the TF levels of asphyxia groups came down obviously,there was no significant between the slight asphyxiated group and the control group(P>0.05);But there was still significant between the severe asphyxia group and the control group(P<0.05).The TP levels of the slight asphyxia group was lower than that of severe asphyxiated group,but there was no significant(P>0.05).There were no significant between the levels of TFPI in the three groups(P>0.05);The TFPI/TF of the asphyxia groups was lower than that in control group(P<0.05),and the TFPI/TF of the slight asphyxia group was a little higher than that of sever asphyxiated group(P>0.05).Conclusion Levels of TF and TFPI changed greatly in asphyxiated newborns,TF continual increased obviously,TFPI increased firstly,but decreased greatly as the hypoxia aggravated;The TFPI/TF decreased.All these changes were positively related with the degree of hypoxia.
