【摘要】 目的探讨大通道Peel-away鞘耻骨上膀胱穿刺造瘘对重度前列腺增生电切术的影响。方法比较造瘘组及非造瘘组重度前列腺增生患者电切手术时间、切除组织量、TUR综合征的发生率、术前术后血钠的指标,观察大通道Peel-away鞘耻骨上膀胱穿刺造瘘在重度前列腺增生电切术中的安全性及有效性。结果与非造瘘组相比,造瘘组电切手术时间延长(p<0.05);切除组织量增多(p<0.05);无1例发生TUR综合征,TUR综合征的发生率明显减低(p<0.05);术前术后血钠值差异无统计学意义(p>0.05)。结论大通道Peel-away鞘耻骨上膀胱穿刺造瘘能避免重度前列腺增生症患者在行经尿道前列腺电切时血钠下降、TUR综合征的发生。同时能保证手术视野清晰。保证了手术安全。更多还原

【Abstract】 Objective To evaluate the influence of suprapubic cystostomy with great channel Peel-away sheath during transurethral resection of prostate(TURP) for the treatment of enlarged benign prostatic hyperplasia.Methods Cystostomy group and non-cystostomy group were compared with the duration of operation,the quantity of resected tissue,the ratio of TUR syndrome,pre-and post-operation serum sodium so as to observe the security and validity of suprapubic cystostomy with great channel Peel-away sheath during transurethral resection prostate(TURP) for the treatment of enlarged benign prostatic hyperplasia.Results Compared with the non-cystostomy group,the cystostomy group underwent longer duration of operation(p<0.05),more quantity of resected tissue(p<0.05),lower ratio of TUR syndrome without single one case(p<0.05).However there were no statistically difference between pre-and post-operation serum sodium in cystostomy group(p>0.05).Conclusion Suprapubic cystostomy with great channel Peel-away sheath during transurethral resection of prostate(TURP) for the treatment of enlarged benign prostatic hyperplasia could keep away from the drop of serum sodium and occurrence of TUR syndrome.Meanwhile it kept the operative visual field clear ensuring the safety.更多还原
