64层螺旋CT灌注成像在乳腺疾病诊断中的应用研究
APPLICATION OF 64-SLICE SPIRAL CT PERFUSION IMAGING IN THE DIAGNOSIS OF BREAST DISEASES
【摘要】 [目的]进行64层螺旋CT灌注成像研究,探讨该技术在乳腺疾病诊断中的应用前景。[方法]选择于2008年5月~2009年5月期间临床检查或钼靶检查怀疑有乳腺疾病的61例女性患者行CT灌注扫描。按照病理将其分为3组:增生病组(n=14)、纤维腺瘤组(n=19)和乳腺癌组(n=20),并将所有患者对侧乳腺定为正常组(n=61)。进行灌注序列bodyperfusion扫描及Perfusion3体部灌注软件处理,计算感兴趣区的血流动力学参数血流量(BF)、平均通过时间、血容量(BV)(MTT)和表面通透性(PS),统计分析不同组间的测量值。[结果]与乳腺癌组比较,正常组BF、BV及PS的差异均有统计学意义(P﹤0.05),MTT的差异无统计学意义(P﹥0.05);正常乳腺组与纤维腺瘤组、增生病组比较,BF及BV的差异均有统计学意义(P﹤0.05),MTT及PS的差异均无统计学意义(P﹥0.05);乳腺癌组与纤维腺瘤组比较,其BF、PS的差异有统计学意义(P﹤0.05),BV及MTT的差异无统计学意义(P﹥0.05);乳腺癌组与增生病组比较,其BF、BV及PS的差异均有统计学意义(P﹤0.05),MTT的差异无统计学意义(P﹥0.05);增生病组与纤维腺瘤组比较,其BF、BV、MTT及PS的差异均无统计学意义(P﹥0.05)。正常组与乳腺癌组分别呈低灌注、高灌注状态,纤维腺瘤组及增生病组均呈较高灌注状态。[结论]64层螺旋CT作为先进的影像检查技术,其灌注成像能从血流动力学和影像学角度为各类乳腺疾病的进行较为准确的诊断。
【Abstract】 [Objective]To study and evaluate multi-slice spiral CT perfusion imaging in the diagnosis of breast diseases the effect and value.[Methods]61 patients through clinical examination or mammography of women with suspected breast disease patients with 64-slice spiral CT perfusion imaging. The pathology types were divided into three groups:hyperplasia group(n = 14),fibroadenoma group(n = 19)and breast cancer group(n = 20),and contralateral breast in all patients as the normal group(n = 61). After perfusion sequence body perfusion body scan and perfusion Perfusion 3 software processing,the hemodynamic parameters were calculated,including area blood flow(BF),mean transit time(MTT),blood volume(BV)and permeability surface(PS),and we analyzed the significant difference among the different groups.[Results]Compared with the breast cancer patients,there was significant difference among BF,BV and PS(P﹤0.05),and there was no significant difference in MTT(P﹥0.05);Compared with the normal mammary gland and fiber adenomas and hyperplasia disease group,there was significant difference among the BF and BV(P﹤0.05),there was no significant different in MTT and PS(P﹥0.05);Compared the breast cancer group with fibroadenomas group,there was significant difference in BF and PS(P﹤0.05),and there was no significant difference between BV and MTT(P﹥0.05);Compared the breast cancer group with the glands group,there was significant difference in the BF,BV,and PS(P﹤0.05),and there was no significant difference in MTT(P﹥0.05);Compared illness group c with fibroadenomas group,there was no significant difference in BF,BV,MTT,and PS(P﹥0.05). Normal group and the breast cancer group SHOWED hypoperfusion,high perfusion,fiber adenoma,and hyperplasia group showed a higher perfusion.[Conclusion]64-slice spiral CT imaging as an advanced technology,its hemodynamics and perfusion imaging could accurate diagnosed the breast disease.
