【摘要】 [目的]探讨经髂腹股沟入路重建钢板固定治疗骨盆骨折的临床疗效。[方法]选取某院自2005年1月～2010年10月治疗的98例不稳定骨盆骨折患者,根据患者意愿随机分为两组,经髂腹股沟入路切开复位内固定术组(50例)为治疗组,外固定手术组(48例)为对照组,术后所有患者均随访6～40个月直至骨折愈合,观察骨折愈合的情况,进行疗效比较分析。[结果]术后经6～40个月的跟踪随访,所有患者均无感染及血管神经损伤等手术并发症,根据Majeed骨盆骨折评价标准,治疗组疗效优30例,良16例,可2例,差2例。总优良率为92.00%;对照组48例患者中优23例,良14例,可7例,差4例,总优良率为77.08%,两者疗效比较差异有统计学意义(P﹤0.05)。[结论]经髂腹股沟入路重建钢板固定治疗骨盆骨折,复位更加直观,手术过程中患者无须变换体位,方便手术操作,避免了医源性损伤,对骨盆的稳定性恢复效果好,致残率低,有重要的临床应用价值。更多还原

【Abstract】 [Objective]To evaluate the ilioinguinal reconstruction plate fixation for the treatment of pelvic fracture patients.[Methods]98 cases of unstable pelvic fractures in our hospital from January 2005-October 2010 were randomly divided into two groups according to the wishes of patients,the ilioinguinal approach open reduction and internal fixation group(50 cases)as treatment group,fixing operation group(48 cases)as the control group,all patients were followed up after 6 to 40 months until the fracture healing,fracture healing situation,the efficacy were compared and analyzed.[Results]After 6 to 40 months follow-up,all patients had no infection and surgical complications such as neurovascular injury,according to Majeed pelvic fracture evaluation criteria,30 cases were in excellent treatment group,good 16 cases and general 2 cases,and poor 2 cases.The excellent and good rate was 92.00%.In control group,23 cases were excellent in 48 patients,good 14 cases,general 7 cases and 4 cases poor,the total excellent and good rate was 77.08%.There was a significant difference in two groups(P﹤0.05).[Conclusion]Through ilioinguinal reconstruction plate fixation for treatment of pelvic fracture,reduction is more intuitive,meanwhile patients needn’t change positions during surgery,which facilitates surgical operation and avoids iatrogenic injury,effectively recovers the stability of the pelvis,with low mutilation and has important clinical value.更多还原
