母脐血管上缺氧诱导因子及其靶基因血红素氧合酶-1的表达与窒息新生儿的相关研究
Correlative study between expressions of HIF and its target genes HO-1 in umbilical vessel and asphyxial newborn
【摘要】 目的观察缺氧诱导因子(HIF)、血红素氧合酶-1(HO-1)在脐血管上的表达,探讨新生儿脐血中HIF-1、HO-1蛋白表达与窒息的相关性。方法选择窒息新生儿45例(轻度窒息新生儿30例,重度窒息新生儿15例),并以健康足月适于胎龄儿15例为对照。于新生儿娩出后立即留取近胎儿处脐带标本,用免疫组织化学法观察新生儿脐血管上HIF-1和HO-1的表达。结果①脐血管上HIF-1主要定位于脐动、静脉血管内皮细胞及血管平滑肌细胞核内。窒息组与对照组比较HIF-1表达明显增加,有显著性差异(P<0.01);轻度窒息组与重度窒息组比较无显著性差异(P>0.05)。②脐血管上HO-1主要定位于脐动、静脉血管内皮细胞及血管平滑肌细胞胞浆。窒息组与对照组比较HO-1表达明显增加,有显著性差异(P<0.01);轻度窒息组与重度窒息组比较无显著性差异(P>0.05)。③窒息组及对照组间的HIF-1和HO-1的表达均呈正相关(P均<0.05)。结论①HIF-1在脐血管上的表达,窒息组明显强于对照组,说明胎儿宫内缺氧缺血诱导HIF-1基因表达,导致HIF-1表达明显增高。②HO-1在脐血管上的表达,窒息组明显强于对照组,是由于胎儿宫内缺血缺氧,诱导HO-1基因表达,同时其上游基因HIF-1表达增强,也导致HO-1表达明显增高。③HIF-1和HO-1表达呈正相关,说明其存在调控关系。HO-1表达在上游基因诱导下增强,说明缺氧状态下机体可反射性启动反应机制。
【Abstract】 Objective It is to observe the expressions of HIF and HO-1 on umbilical vessel and to approach the correlation between the expressions of HIF-1 and HO-1 in cord blood of newborn and asphyxia.Methods Forty-five asphyxial newborn were chosen in which 30 cases were lower grade and 15 cases were severe grade.Meanwhile,15 healthy full term newborns were chosen as control.The umbilical cords near fetus were kept immediately after the newborns were expulsion.The expressions of HIF-1 and HO-l in cord blood vessels were observed with immunohistochemistry method.Results ①HIF-1 mainly expressed in endothelial cells of cord artery and cord vein as well as in nucleus of vascular smooth muscle cell.The expression of HIF-1 in asphyxia group was higher than that in control group and the difference was significant(P<0.01).There was no significant difference between lower grade asphyxia group and severe grade asphyxia group(P>0.05).②HO-1 mainly expressed in endothelial cells of cord artery and cord vein as well as in hyalomitome of vascular smooth muscle cell.The expression of HO-1 in asphyxia group was higher than that in control group and the difference was significant(P<0.01).There was no significant difference between lower grade asphyxia group and severe grade asphyxia group(P>0.05).③The expressions of HIF-1 and HO-1 in asphyxia group and control group all showed positive correlation(all P<0.05).Conclusion ①The expression of HIF-1 on umbilical vessel in asphyxia group is obviously higher than that in control group,which explains that fetus sufferring hypoxia in uterus inducing the enhancement expression of HIF-1.②The expression of HO-1 on umbilical vessel in asphyxia group is obviously higher than that in control group,because that fetus sufferring hypoxia in uterus and meanwhile its upstream gene HIF-1 expresses enhancement inducing the enhancement expression of HO-1.③The expressions of HIF-1 and HO-1 show positive correlation explains that regulation exists.The expression of HO-1 enhances induced by its upstream gene explains that the organism can be reflective of the response mechanism under anerobic cendition.
