血清胱抑素C与脑梗死发病相关性的临床研究

【摘要】 目的研究血清胱抑素C(Cys-C)与脑梗死的相关性。方法回顾性分析比较180例脑梗死患者与112例对照组患者的血清Cys-C水平及相关临床资料。结果①脑梗死组患者血清Cys-C水平(0.95±0.23 mg/L)较对照组(0.76±0.14 mg/L)明显增高(P<0.01);②血清Cys-C水平与患者性别有关,且与年龄(r=0.180,P=0.016)、同型半胱氨酸(r=0.257,P=0.001)呈正相关,与高密度脂蛋白(r=-0.219,P=0.003)呈负相关;③Logistic回归分析显示,年龄(OR=1.063,P<0.001)、吸烟(OR=8.178,P<0.001)、高血压(OR=7.649,P<0.001)、糖尿病(OR=4.798,P=0.011)及血清Cys-C(OR=1.675,P=0.007)均是脑梗死的危险因素。经控制其他危险因素后,血清Cys-C仍与脑梗死密切相关(r=0.215,P<0.01)。结论血清Cys-C水平的升高是脑梗死的独立危险因素。

【Abstract】 Objective To investigate the correlation between serum cystatin C(Cys-C) level and cerebral infraction.Methods We made a retrospective study on serum Cys-C and related clinical data of 180 patients with cerebral infarction and 112 patients with other diseases.Results ① The level of serum Cys-C was significantly higher in the case group(0.95±0.23mg/L) than in the control group(0.76±0.14mg/L)(P<0.01).② Serum Cys-C level of the patients with cerebral infarction was related to sex.It had a positive correlation with age(r=0.180,P=0.016) and homocysteine level(r=0.257,P=0.001),but a negative correlation with high-density lipoprotein cholesterol level(r=-0.219,P=0.003).③ Logistic regression analysis indicated that age(OR=1.063,P<0.001),smoking(OR=8.178,P<0.001),hypertension(OR=7.649,P<0.001),diabetes(OR=4.798,P=0.011) and serum Cys-C(OR=1.675,P=0.007) were risk factors for cerebral infarction.After controlling other risk factors,Cys-C was still related to cerebral infarction(r=0.215,P<0.01).Conclusion The increase of serum Cys-C is an independent risk factor for cerebral infarction.
