原发性病理性十二指肠胃反流患者胃黏膜TNF-α和COX-2的表达及其与细胞凋亡的关系
Study on the expression of TNF-α、COX-2 and the relation with apoptosis of gastric mucosal cells in primary pathological duodenogastric reflux
【摘要】 【目的】研究原发性病理性十二指肠胃反流(duodenogastric reflux,DGR)患者胃黏膜组织中的肿瘤坏死因子-α(tumornecrosis factor-α,TNF-α)、环氧合酶-2(cyclooxygenase-2,COX-2)表达状态及其与细胞凋亡的关系,探讨原发性病理性十二指肠胃反流的发病机理。【方法】应用脱氧核糖核酸转移酶介导的缺口末端标记技术和免疫组化方法分析86例原发性病理性DGR和42例正常对照胃黏膜细胞凋亡、肿瘤坏死因子-α和环氧合酶-2的表达。【结果】原发性病理性DGRTNF-α表达指数显著高于正常对照组(P<0.05),Hp阳性亚组TNF-α表达指数显著高于Hp阴性亚组(P<0.05)。原发性病理性DGR组COX-2表达指数显著高于正常对照组(P<0.05),Hp阳性和Hp阴性亚组之间COX-2表达指数无显著性差异(P>0.05)。原发性病理性DGR组TNF-α的表达与凋亡指数呈正相关(r=0.705,P=0.000),COX-2的表达与凋亡指数呈正相关(r=0.584,P=0.000)。【结论】原发性病理性DGR胃黏膜TNF-α和COX-2表达增加,诱导胃黏膜细胞凋亡。Hp感染未加重原发性病理性DGR所致胃黏膜细胞凋亡反应。
【Abstract】 【Objective】 To Study on the expression of tumor necrosis factor-α(TNF-α)、cyclooxygenase-2(COX-2) and their relation with apoptosis of gastric mucosal cells in primary pathological duodenogastric reflux,and investigate the mechanism of primary pathological duodenogastric reflux.【Methods】Apoptosis cells in gastric mucosa were quantitated by terminal deoxynucleotidyl transferase mediated dUTP nick end-labelling and immunohistochemical staining was used to detect the expression of TNF-α and COX-2 in gastric mucosa of 86 cases in primary pathological duodenogastric reflux group and 42 cases in normal group.【Results】 The expression index of TNF-α in primary pathological duodenogastric reflux group was significantly higher than that in normal group(P < 0.05).The expression index of TNF-α in Hp positive subgroup was significantly higher than that in Hp negative subgroup(P < 0.05).The expression index of COX-2 in primary pathological duodenogastric reflux group was significantly higher than that in normal group(P < 0.05).The difference of COX-2 between Hp positive and Hp negative subgroup showed no significance(P > 0.05).The expression of TNF-α in primary pathological duodenogastric reflux group was positively correlated with AI(r = 0.705,P = 0.000),The expression of COX-2 in p primary pathological duodenogastric reflux group was positively correlated with AI(r = 0.584,P = 0.000).【Conclusion】 The expression of TNF-α and COX-2 in gastric mucosa of primary pathological duodenogastric reflux is increased,which can induce gastric mucosa cell apoptosis.Hp infection can not aggravate gastric mucosa cell apoptosis caused by primary pathological duodenogastric reflux.
