高血压合并葡萄糖调节受损患者血浆妊娠相关蛋白A水平变化及相关因素研究
Study of correlations among plasma pregnancy associated plasma protein-A relevant factors in hypertension patients with impaired glucose regulation
【摘要】 目的探讨高血压合并葡萄糖调节受损(IGR)患者血浆妊娠相关蛋白A(PAPP-A)水平变化及其与血压、WHR、血糖、血脂、血浆胰岛素、高敏C反应蛋白(hs-CRP)水平等的关系。方法酶联免疫吸附法测定IGR患者(IGR组)、高血压合并IGR患者(HT+IGR组)和正常人(NC组)血浆PAPP-A水平,分析其与血压、BMI、WHR、血脂、HOMA-IR、hs-CRP等的关系。结果 HT+IGR组血浆PAPP-A水平明显高于IGR组和NC组(P<0.01)。血浆PAPP-A水平与年龄、体重、腰围、臀围、舒张压(DBP)、体内脂肪百分比(FAT%)、BMI、WHR、TG、FFA、FPG、2hPG、FIns2、hPIns、HOMA-IR、hs-CRP、HbAlc呈正相关。多元回归分析结果表明腰围、DBP、2hPIns、HbAlc是影响血浆PAPP-A水平的独立相关因素。结论血浆PAPP-A水平的改变与糖脂代谢和血压等有关,并可能参与了胰岛素抵抗及高血压合并糖尿病的发生发展。
【Abstract】 Objective: To investigate the changes of plasma pregnancy associated plasma protein-A(PAPP-A) level and the relationship among plasma PAPP-A level and blood pressure,waist hip ratio(WHR),blood lipids,blood glucose,plasma insulin,plasma highly sensitive C-reactive protein(hs-CRP) levels as well as other factors in hypertension patients with impaired glucose regulation(IGR).Methods: Plasma PAPP-A levels were assayed by ELISA in the IGR(IGR),hypertension patients with IGR(HT+IGR) and normal controls(NC).The relationship between plasma PAPP-A levels and blood pressure,BMI,WHR,blood lipids,HOMA-insulin resistance index(HOMA-IR) and hs-CRP was also analyzed.Results: Plasma PAPP-A level in HT+IGR was significantly increased,as compared with IGR and NC [(21.22±4.12) vs(8.21±4.22) and(7.97±4.36) mIU/L,both P<0.01].Plasma PAPP-A level was correlated positively with age,body weight,waist circumference(WC),hip circumference,DBP,visceral fat%(FAT%),BMI,WHR,TG,FFA,FPG,2hPG,FIns,2hPIns,HbAlc,HOMA-IR,hs-CRP(r=0.197,r = 0.249,r = 0.328,r = 0.201,r = 0.268,r =0.302,r = 0.337,r = 0.238,r = 0.252,r = 0.163,r =0.168,r = 0.211,r = 0.182,r = 0.209,r =0.191,r = 0.210,r=0.345,respectively P<0.01 and P<0.05).Multiple regression analysis showed that WC,DBP,2hPIns and HbAlc were independent related factors influencing plasma PAPP-A level.Conclusion: The changes of PAPP-A levels are associated with glucose and lipid metabolism and blood pressure,and it may contribute,in part,to the pathogenesis of insulin resistance(IR) and hypertension with diabetes complicated.
