【摘要】 目的探讨鼻咽癌首程放疗后鼻咽腔溃疡形成的原因、诊断、治疗以及预后。方法对35例首程放疗后出现鼻咽腔溃疡的患者,行常规放疗20例,调强放疗15例,35例均行化疗,经电子鼻咽镜直视下行坏死物清除术、消炎治疗及鼻腔冲洗。结果 20例基本愈合,8例临床症状如头痛、异味明显改善,4例改善不明显,1例颅内感染,2例颅底骨坏死。结论鼻咽腔巨大深溃疡与肿瘤T分期、放疗、化疗有关,同时与肿瘤侵犯位置一致,严重鼻咽腔溃疡的患者预后差。更多还原

【Abstract】 Objective To analyse reason,diagnosis,treatment and prognosis of nasopharyngeal ulcer after first radiotherapy for nasopharyngeal carcinoma(NPC).Methods From Jan.2008 to Jul.2010,35 NPC patients with nasopharyngeal ulcer after first radiotherapy were treated in Zhejiang Cancer Hospital.Twenty patients received conventional radiotherapy and 15 received intensity modulated radiotherapy,and all the cases received chemotherapy.The patients were treated with endoscopy-guided debridement and systemic anti-inflammatory treatment and rhinal wash.Results Twenty patients with nasopharyngeal ulcer were cured,clinical symptoms such as foul odor and headache were alleviated in 8 patients,but 4 patients were bad,encephalic infection occurred in 1 patient and skull bone putrescence occurred in 2 patients.Conclusions Nasopharyngeal ulcer is related to T stage,radiotherapy,chemotherapy and consistent with tumor volume.The prognosis of NPC patients with severe nasopharyngeal unlcer is relatively poor.更多还原
