血清白蛋白的变化与急性重型颅脑损伤颅内出血的关系研究

【摘要】 目的探讨急性重型颅脑损伤颅内出血患者血清白蛋白(Alb)水平变化及其临床意义。方法选择本院收治的急性重型颅脑损伤颅内出血患者54例为研究组,另选择同期于本院健康体检者30例为正常对照组,采用日本AU-2700型全自动生化分析仪检测各组不同时间段血清Alb水平。结果研究组重型与轻型患者入院后第1、3、7、14 d血清Alb水平较正常对照组明显降低(P<0.05),重型患者入院第7、14 d的血清Alb水平较轻型患者明显降低,差异有统计学意义(P<0.05)。根据重型颅脑损伤颅内出血患者出院时的预后分为死亡组37例,存活组17例。死亡组入院后第1 d血清Alb水平与存活组差异无统计学意义(P>0.05),但入院后3、7、14 d血清Alb水平明显低于存活组(P<0.05)。结论血清白蛋白的动态观察对评估急性重症颅脑损伤颅内出血患者的病情发展、疗效和预后具有重要临床价值。

【Abstract】 Objective To explore the change and clinical significance of plasma Alb in acute severe brain injury patients with intracranial bleeding. Methods Fifty-four acute severe brain injury patients with intracranial bleeding were selected as study group and 30 healthy physical examinees were served as control group.And AU-2700 Automatic Biochemical Analyzer was used to measure plasma Alb at the different time points. Results The levels of plasma Alb in the severe and slight patients of study group on 1st,3rd,7th,and 14th days after admission were significantly lower than those in control group(P<0.05).The levels of plasma Alb in severe patients of study group on 7th and 14th days after admission were significantly lower than those in slight patients of study group(P<0.05),and the difference was statistically significant.There were 37 death cases and 17 survival cases according to the prognoses of acute severe brain injury patients with intracranial bleeding. No statistically significant difference was found in the levels of plasma Alb on 1st day after admission between death cases and survival cases(P>0.05),but the levels of death cases on 3rd,7th,and 14th days after admission were significantly lower than those in survival cases(P<0.05). Conclusions Dynamically observing the level of plasma Alb is of great clinical value for assessing the disease development,therapeutic efficacy and prognosis of acute severe brain injury patient with intracranial bleeding.
