【摘要】 目的观察卡前列甲酯栓联合催产素防治高危产妇剖宫产术后出血的临床效果。方法选择我院妇产科2008年1月-2010年12月间200例行剖宫产手术的高危产妇,随机分为研究组和对照组,每组100例,研究组产妇在术中打开腹膜时给予卡前列甲酯栓1 mg舌下含化,胎儿娩出后给予宫体肌肉注射催产素20 U;对照组产妇仅在胎儿娩出后给予宫体肌肉注射催产素20 U。观察术中、术后2 h内产妇出血量及出血总量。结果研究组100例高危产妇术中出血量为(302±178)ml,产后2 h内出血量为(108±78)ml,出血总量为(408±207)ml;对照组100例高危产妇术中出血量为(428±206)ml,产后2 h内出血量为(141±92)ml,出血总量为(558±285)ml。两组间差异有统计学意义(P<0.05)。研究组出血总量≥1 000 ml的产妇有3例(3%),对照组出血总量≥1 000 ml的产妇有6例(6%)。结论卡前列甲酯栓联合催产素能有效的防治高危产妇剖宫产术后出血,其作用优于单独使用催产素,值得临床推广。更多还原

【Abstract】 Objective To observe the therapeutic efficacy of carboprost and oxytocin on prevention of high-risk maternal postplartum hemorrhage after cesarean section. Methods We selected 200 high-risk parturient women who took in the caesarean birth in the People’s Hospital of Yongqing County from January 2008 to December 2010.They were randomly divided into research group and control group(each n=100).The women in research group were given 1 mg of carboprost and 20 U of oxytocin.The women in control group were only received 20 U of oxytocin after fetal childbirth.The intraoperative bleeding,blood loss during the first 2 postoperative hours,and the total bleeding volume were observed. Results The intraoperative bleeding of 100 cases of research group was(302±178) ml,blood loss during the first 2 postoperative hours was(108±78) ml,and the total bleeding volume was(408±207) ml.The intraoperative bleeding of 100 cases of control group was(428±206) ml,blood loss during the first 2 postoperative hours was(141±92) ml,and the total bleeding volume was(558±285) ml.There were statistically significant difference between the two groups(P<0.05).There were 3 patients(3%) in research group and 6 patients(6%) in control group whose total bleeding volume was ≥ 1,000 ml. Conclusions Carboprost combined with oxytocin can effectively prevent high-risk maternal bleeding after cesarean section,and its function is superior to oxytocin used alone.It is worthy of clinical promotion.更多还原
