【摘要】 目的比较腔镜腋窝淋巴结清扫术和常规手术近期临床疗效的差异。方法回顾性分析2010年1-9月在中南大学湘雅三医院普外二科收治的56例行腋窝淋巴结清扫术的乳腺癌患者,比较腔镜组和常规组的近期临床疗效。结果与常规组相比较,腔镜组平均手术出血量较少(44.3 ml vs 89.4 ml,P<0.05)、平均手术时间较长(92.5 min vs70.5 min,P<0.05),但平均淋巴结数和平均阳性淋巴结数差异无统计学意义(16.1 vs 16.9,3.5 vs 3.8;P>0.05)。术后并发症中腔镜组有一例出现术后伤口感染,常规组有两例出现术后淋巴瘘。结论乳腺癌腔镜腋窝淋巴结清扫术与传统手术相比较不但同样能彻底的清扫腋窝淋巴结,同时具有微创、美容、并发症少等优势,值得推广应用。更多还原

【Abstract】 Objective To compare the recent clinical effects between the laparoscopic axillary lymph node dissection and conventional axillary lymph node dissection. Methods Clinical data of 56 breast cancer patients underwent axillary lymph node dissection in the Second Department of General Surgery,the Third Xiangya Hospital from January to September,2010 were retrospectively analyzed.The recent clinical effects were compared between laparoscopic group and conventional group. Results The average amount of bleeding in laparoscopic group was less than that in conventional group(44.3ml vs 89.4ml,P<0.05).The average operation time of conventional group was shorter than that of laparoscopic group(70.5 min vs 92.5 min,P<0.05).No statistically significant difference was found in the average number of lymph nodes and the average number of positive lymph nodes between conventional group and laparoscopic group(16.9 vs 16.1,3.8 vs 3.5;P>0.05).One case of postoperative wound infection was found in laparoscopic group.Lymphatic fistula occurred in two cases of conventional group. Conclusions As compared with conventional axillary lymph node dissection,laparoscopic axillary lymph node dissection not only has the same effect in lymph node dissection but also has the advantages of minimal invasion,cosmetic surgery,and less complications.This method is worthy to be popularized in clinical application.更多还原
