【摘要】 目的 分析肝脏血管平滑肌脂肪瘤(hepatic angiomyolipoma,HAML)的CT特征,提高诊断准确性。方法 在PACS影像诊断系统上,回顾性分析经手术病理证实的12例HAML的CT表现,记录病灶部位、大小、边缘、密度及强化。结果(1)12例病灶均无包膜;(2)病灶内含脂肪密度11例,其中以脂肪密度为主的有3例,其余8例含少量脂肪密度。在降低窗位、增加窗宽时脂肪密度内可见网格状、团絮状影;(3)HAML呈现早期强化、延迟强化的特征,增强脂肪密度区CT值有明显提高,延迟扫描部分病例脂肪密度区形态改变;5例显示增粗的供血血管。结论 病灶内含脂肪密度、早期强化及延迟强化、增粗的供血血管是HAML的CT特征,发现上述征象有助于诊断。更多还原

【Abstract】 Objective To analyze the CT features of hepatic angiomyolipoma(HAML),so that to improve the diagnostic accuracy of this disease.Methods CT findings of 12 patients with HAML proven by surgery and pathology were retrospectively analysed at PACS system.The location,size,margin,density and enhancement features of the lesions were recorded and analysed comparatively with pathological findings.Results(1)All cases were no capsule;(2)The regions with fat density appeared in 11 cases,among them,mainly fat density zone were appeared in 3 cases,for the rest a little fat content was seen.Grid or flocculation-like appearances were showed when lowering window level and broadening window width;(3)12 cases were marked enhancement in the arterial and portal phase,the CT value of the regions with fat density regions were obviously increased after administration of contrast medium,and in these cases,the shape of fat zones was changed in the delay scan,and the vascular thickening were seen in 5 cases.Conclusion Fat density,abnormal vessel in the lesions,and marked enhancement in the arterial and portal phase,even on the delay phase are the typical features of HAML on the CT images.更多还原
