【摘要】 目的 探讨实时监控技术(Smart Prep)在小肝癌多层螺旋CT动态增强扫描中的应用价值。方法 采用GE Light Speed 64排VCT对40例临床怀疑小肝癌的患者进行分组CT动态增强检查。A组20例应用实时监控技术(阈值为100 HU),B组20例为应用传统的三期动态扫描(动脉期为25~30 s、门脉期65 s、延迟期2 min)。结果 A组均能获得最佳动脉期图像,17例诊断正确,仅1例误诊。而B组只有7例获得最佳动脉期图像,仅10例诊断正确,3例误诊。A、B2组肝动脉CT值、病灶CT值差异具有统计学意义(P<0.01)。结论 Smart Prep技术对提高小肝癌的检出率有重要价值。

【Abstract】 Objective To investigate the applied value of real-time monitored-triggered technique(smart prep) during multiple spiral CT dynamic scanning in detecting small hepatocellular carcinoma(SHCC).Methods 40 cases of clinical suspected of small liver cancer were divided in two groups(20 cases in each group) and underwent contrast-enhanced CT examination using GE Light Speed 64-row VCT.Smart prep threshold value is 100 HU) in group A and traditional three phase scanning(artery phase at 25~30 s delayed,portal vein phase at 65 s delayed and delay phase at 2 min delayed) in group B were served during CT scanning.Results The arterial phase images in group A were all fine,and 17 cases were accurately diagnosed with SHCC confirmed by pathology,while the arterial phase smages only in 7 cases in group B were fined,and only 7 cases with SHCC were correctly diagnosed.Conclusion By monitoring the concentration fluctuation of contrast in the aorta for real time,the smart prep technique can improve the detection of small hepatocellular carcinoma.
