椎基底动脉扩张延长症的多层螺旋CT血管成像表现及应用价值

【摘要】 目的 探讨多层螺旋CT血管成像(MSCTA)在椎基底动脉扩张延长症(VBD)中的应用价值及临床意义。方法 对33例VBD患者的影像资料进行回顾性分析,并与对照组(40例)比较后循环血管狭窄性病变和脑梗死的发生率。结果 33例扩张的椎基底动脉直径4.8~14.3 mm,迂曲的基底动脉和椎动脉颅内段长度分别为30.3~41.7 mm和23.9~42.2 mm,33例中发现大脑后动脉及椎动脉狭窄或闭塞11例,后循环供血区腔隙性脑梗死15例,大面积脑梗死3例;对照组40例中,椎动脉及大脑后动脉闭塞各1例,椎基底动脉和(或)大脑后动脉狭窄22例,后循环供血区腔隙性脑梗死7例,大面积脑梗死3例,2组间后循环供血区腔隙性脑梗死发生率差异有统计学意义(P<0.05)。结论 VBD可能是后循环供血区腔隙性脑梗死的重要危险因素;MSCTA结合原始图像可以清晰显示VBD及其颅内伴随病变。

【Abstract】 Objective To evaluate the value and clinical significance of multi-slice spiral CT angiography(MSCTA) in vertebrobasilar dolichoectasia(VBD).Methods Image data in 33 cases of VBD were retrospectively analyzed,and the incidences of stenosis of the posterior cycle of vascular and cerebral infarction were compared with that of control group(40 cases).Results The diameter of dilated vertebrobasilar artery in 33 cases ranged from 4.8 mm to 14.3 mm,and the lengths of intracranial segment of circuitous basilar artery and vertebral artery ranged from 30.3 mm to 41.7 mm,and from 23.9 mm to 42.2 mm,respectively.Among the33 cases of VBD,there were 11 cases of stenosis or occlusion of posterior cerebral artery and vertebral artery,15 cases of lacunar infarction in posterior cerebral circulation area,and 3 cases of large infarction area.Among the 40 cases in control group,there were 1case with occlusion of the vertebral artery and 1 case with occlusion of posterior cerebral artery,22 cases with stenosis of vertebral basilar and posterior cerebral artery,7 cases with lacunar infarction of posterior circulation,3 cases with large cerebral infarction.There was significant difference in the incidence in lacunar infarction between the two groups(P<0.05).Conclusion VBD may be an important risk factor of lacunar infarction in posterior cerebral circulation.MSCTA can clearly show intracranial VBD and its accompanying diseases.
