【摘要】 目的 探讨胰腺囊性病变的CT表现及鉴别诊断。方法 采用经手术病理证实的45例胰腺囊性病变(假性囊肿24例、浆液性囊腺瘤7例、黏液性囊腺瘤6例、黏液性囊腺癌2例、实性一假性乳头状瘤6例)的CT影像资料,并结合临床及相关病史做C T回顾性分析。结果 24例假性囊肿均发生于胰腺内或胰腺周围组织间隙.20例囊壁薄,4例壁厚,2例囊内伴团块状高密度出血灶。7例浆液性囊腺瘤.3例位于胰头部,体尾部各2例,5例为呈蜂窝状或海绵状改变,2例呈单房表现。8例黏液性囊腺瘤(癌),3例位于胰体部,5例位于胰尾部,多呈类圆形及椭圆形,3例见钙化,5例伴壁结节,2例病理诊断为囊腺癌.1例腹膜后淋巴结肿大。6例实性-假乳头状瘤,4例位于胰体部,2例位于胰尾部,4例囊实成分相仿,1例以囊性成分为主,1例以实性成分为主,2例见钙化。结论 假性囊肿、囊腺瘤及实性-假乳头状瘤是胰腺较常见囊性病变,胰腺囊性病变在CT表现上具备很多共性,CT诊断必须密切结合临床及相关病史。更多还原

【Abstract】 Objective To study CT feat ures and differential diagnosis of the cystic lesions of the pancreas.Methods CT data in 45cases with pancreatic cystic lesions confirmed by surgery and pathology(24 cases of pseudocyst,serous cystadenoma in 7 cases,6 cases of mucinous cystadenoma,2 cases of mucinous cystadenocarcinoma and 6 cases of solid-pseudopapillary tumor) were retrospectively analysed in combination with clinical history.Results 24 cases of pancreatic pseudocyst occurred in the pancreas or the space around the pancreas,cyst walls of lesions were thin in 20 and thickness in 4,massive hemorrhage of high density inside the lesions was seen in 2 cases.7 cases of serous cystadenoma,the lesions located in the pancreatic head in 3,body and tail in 2 cases respectively,5 cases showed honeycomb-like or sponge-like changes,2 cases showed unilocular.8 cases of mucinous cystadenoma(cancer),the lesions were round and ellipse,located in the pancreatic body in 3 and the pancreatic tail in 5,there were 3 cases with calcification,5cases with mural nodules,two cases with cystic adenocarcinoma,1 case with retroperitoneal lymphadenectasis.In the six cases of solid-pseudopapillary tumor,4 lesions occurred in the pancreatic body and 2 occurred in pancreatic tail,4 had the same volume of cystic and solid components,the lesions were cystic mainly in one and solid mainly in one,two had calcification inside the lesions.Conclusion Pesudocyst,cystadenoma,and solid-pseudopapillary tumor are common seen in pancreatic systic lesions,CT features of pancreatic cystic lesions are similarities,CT diagnosis of them need combine with the clinical data.更多还原
