布加综合征MSCT血管造影扫描技术优化
Technique Optimization of MSCT Angiography in Budd-Chiari Syndrome
【摘要】 目的 探讨布加综合征合理的MSCT血管造影扫描技术。方法 随机选取82例临床或超声诊断为布加综合征而行MSCT血管造影检查者,分为实验组和对照组;实验组36例,采用新扫描方法,对照组46例,采用传统肝3期扫描方法;原始数据采用容积再现、最大密度投影、最小密度投影及多平面重组技术进行后处理,观察血管内造影剂均匀度及血管显示情况,分析门静脉、肝静脉及下腔静脉最佳扫描时间,并对比2组门静脉、肝静脉图像质量。结果 实验组除2例门静脉内造影剂稍有不均,余造影剂混合均匀,门静脉分支达6~9级:门静脉触发后延迟22~31 s(95%置信区间)显示肝静脉、下腔静脉最好,肝静脉分支达4~7级。与传统肝3期扫描相比较,二者对于门静脉显示效果无明显差异(P>0.05),对于肝静脉显示有明显差异(P<0.05)。结论布加综合征患者行腹部M SCT血管造影检查时,采用新方法较之传统3期扫描方法有很大优势,可有效降低造影剂用量和辐射剂量。
【Abstract】 Objective To explore the reasonable method of MSCT angiography scanning technique in diagnosis of Budd-Chiari syndrome(BCS).Methods Eighty-two cases of BCS diagnosed with clinical or ultrasound were randomly divided into two groups(experimental group and control group).Thirty-six cases in experimental group and 46 cases in control group were analyzed by the new scanning techniques and the traditional tri-phase scanning technique,respectively.The original CT data were reconstructed using3D reprocessing technology including volume rendering(VR),maximum intensity projection(MIP),the minimum intensity projection(MinIP) and multiplanar reformation(MPR).The image quality of portal and hepatic vein(PV,HV) between two groups were compared,and the intravascular contrast evenness and the optimal scanning time of PV,HV and inferior vena cava(IVC) were analyzed.Results Intravascular contrast agent of experimental group cases were all equally distributed except 2,the small branches of portal vein(6-9 segments) could be clearly showed.Scanning after delay time ranged between 22-31 s(95% confidence interval)HV and IVC could be clearly showed.Small branches of HV(4-7 segments) could also be clearly showed.There were no obvious differences(P>0.05) for PV between two groups,but was of obvious difference(P<0.05) with hepatic vein.Conclusion MSCT angiography in diagnosing BCS using the new scanning techniques is superior to the traditional tri-phase scanning technique,which can reduce the dose of contrast and radiation effectively.
