【摘要】 目的 分析不同CT窗技术对肺穿刺活检引导的价值。方法 回顾性分析同一工作组行CT引导下肺穿刺活检的连续性242病例。所有病例按使用不同的窗技术行穿刺者分为2组,单窗(纵隔窗)45例(A组)和多窗(肺窗+纵隔窗+中间窗)197例(B组)。对比分析统计2组间穿刺结果阳性率、气胸及肺出血发生率。结果 A组穿刺结果阳性率88.9%(40/45),气胸发生率22.2%(10/45),肺出血发生率20.0%(9/45)。B组穿刺结果阳性率91.9%(181/197),气胸发生率9.1%(18/197),肺出血发生率5.1%(10/197)。B组肺穿刺活检的气胸和肺出血发生率均明显低于A组,P<0.05。A、B2组的穿刺结果阳性率差异无显著性,P>0.05。结论 联合运用多窗CT引导技术较单一纵隔窗引导技术能有效降低经皮肺穿刺活术的气胸及出血并发症。更多还原

【Abstract】 Objective To determine the value of the different window settings in percutaneous puncture biopsy of lung lesions under CT guidance.Methods 236 patients who underwent CT guidance percutaneous puncture biopsy of lung lesions by the same workgroup were retrospectively analysed.The patients were divided into two groups according to use of different widow settings during biopsy,group A in 45 cases using single-window settings and group B in 197 using multi-window settings(lung window,mediastinum window and median window).The positive rates of biopsy,the incidences of pneumothorax and pulmonary hemorrhage were analysed comparatively.Results The positive rate of biopsy,the incidences of pneumothorax and pulmonary hemorrhage were 40/45(88.9%),10/45(22.2%) and 9/45(20.0%) in group A,181/197(91.9%),18/197(9.1%) and 10/197(5.1 %) in group B,respectively.The incidences of pneumothorax and pulmonary hemorrhage in group B were significantly lower than that in group A(P <0.05).However,there was no significant difference in diagnostic accuracy between two groups(P>0.05).Conclusion By using multiple adjusting window settings can effectively reduce the complications(pneumothorax and pulmonary hemorrhage) during CT guidance percutaneous puncture biopsy in comparison with by using simple mediastina window.更多还原
