【摘要】 目的 研究鼻咽癌放疗后局部复发瘤（local recurrent nasopharyngeal carcinoma,后称复发瘤）各CT灌注参数阈值及其诊断价值。方法 对病理证实的31例复发瘤和病理或随访证实的26例鼻咽癌放疗后局部纤维化患者（后称纤维化组）进行鼻咽部病灶区CT灌注成像扫描,以颈外动脉-下颌后静脉作为感兴趣血管测量各CT灌注参数值并进行统计学分析,确定复发瘤各CT灌注参数阈值及其敏感性和特异性。结果 复发瘤的血流量（BF）、血容量（BV）、平均通过时间（MTT）和表面通透性（PS）阈值分别为347.02 ml·min^-1·g^-1、13.74 ml/100 g、1.82 s、15.39 ml·min^-1·g^-1,诊断敏感性分别为90.30%、83.90%、66.70%、93.50%,特异性分别为93.90%、97.00%、60.00%、88.50%。结论 鼻咽癌MSCT灌注阈值对鼻咽癌局部复发瘤定性诊断具有重要价值。更多还原

【Abstract】 Objective To study the threshold of multi-slice CT（MSCT） prefusion parameters and its diagnostic value in the local recurrent nasopharyngeal carcinoma（NPC） post-radiotherapy.Methods A total 57 patients including 31 patients with local recurrence tumor and 26 patients with local fibrosis post-radiotherapy underwent CT perfusion.The perfusion parameters of the lesions were analysed by statistics and perfusion parameters threshold was also calculated.The sensitivity and specificity were used to evaluate the diagnostic value of perfusion parameters threshold in the local recurrent tumor of NPC.Results The thresholds of perfusion parameters including BF,BV,MTT and PS in the local recurrence tumor of NPC were 347.02(ml·min^-1·100g^-1),13.74 ml/100g,1.82 s and 15.39(ml·min^-1·100g^-1),respectively.The sensitivity and specificity of diagnosis in local recurrence tumor with BF,BV,MTT and PS were 90.30%,83.90%,66.70% and 93.50%,and 93.90%,97.00%,60.00% and 88.50%,respectively.Conclusion The thresholds of MSCT perfusion in local recurrent nasopharyngeal carcinoma post-radiotherapy is helpful for qualitative diagnosis更多还原
