肺部混杂性磨玻璃密度结节的MDCT表现

【摘要】 目的 分析肺部混杂性磨玻璃密度结节(mGGO)的MDCT表现。方法 分析53例经病理或抗炎治疗后证实的mGGO的基本临床资料、病灶大小、CT值及MDCT表现(病灶形态、边缘形态、界面、内部结构、邻近结构),用χ~2检验、Fisher确切概率法、非参数Mann-Whitney U检验分析良恶性mGGO之间是否存在差异。结果 53例mGGO中恶性40例,良性13例。对于基本临床资料、病灶大小及CT值在良恶性mGGO之间均没有统计学差异(P均>0.05)。病灶形态(圆形/类圆形、不规则形)、边缘形态(分叶)、界面、内部结构(空泡征)、邻近结构(胸膜凹陷征)在良恶性mGGO之间均存在明显差异(P均<0.05)。mGGO内GGO含量、实性部分和GGO的分界是否清楚在良恶性之间均没有统计学差异(P=0.507 vs 0.181>0.05);mGGO内GGO含量在腺癌和细支气管肺泡癌2种病理类型之间也没有差异(P=0.726>0.05)。结论 mGGO的MDCT征象:圆形/类圆形、分叶、清楚毛糙的界面、空泡征、胸膜凹陷征是诊断恶性的重要征象。

【Abstract】 Objective To study multi-detector computed tomography features of pulmonary mixed ground-glass opacity(mGGO) nodules.Methods Clinical data,the size,attenuation value and MDCT findings(shape,margin,interface,internal characteristics,adjacent structure) of the lesions in 53 cases with mGGO confirmed by pathology or anti-inflammatory therapy were analyzed.The differences between benign and malignant mGGO were analyzed by using the Fisher exact test or Mann-Whitney U test.Results 53 mGGOs included 40 malignant nodules and 13 benign nodules.In terms of clinical data,the size and attenuation value of the lesions were no statistical differences between benign and malignant mGGO.MDCT findings of round shape,lobulation,welldefined but coarse interface,vacuole sign and pleural indentation sign were significantly different between benign and malignant mGGO.The percentage of GGO areas,well-defined interface of GGO and solid were no significant difference between benign and malignant mGGO(P=0.507 vs.0.181>0.05).No difference was found in the percentage of GGO areas between adenocarcinoma and bronchioalveolar carcinoma.Conclusion MDCT features including round shape,well-defined but coarse interface,lobulation,vacuole sign and pleural indentation sign are of very important value in diagnosing malignant mGGO.
