【摘要】 目的用"新九分法"估计烧伤面积已沿用多年,它具有简捷、方便等优点,但缺肢患者的烧伤面积如何估计,尚无确切方法,若用"新九分法"估计,势必出现低估现象;本研究为评估缺肢患者的实际烧伤面积(RBSA)提供了计算公式。方法以"新九分法"评估参数作为基准,分别估计缺肢患者缺失部位面积和烧伤区域面积的百分数(%),把缺肢后的自身体表总面积(TBSAs)视为100%,经数学处理,找出其规律性,计算用"新九分法"评估的每1%烧伤面积(BSA)应增补的百分数,得出实际烧伤面积(RBSA)计算公式。结果缺肢患者的实际烧伤面积(%TBSAs)=估计烧伤面积百分数×(1+缺失面积百分数÷100);儿童下肢缺失者,因其下肢所占百分数小于成年人,应根据"新九分法"计算出缺失下肢的百分数,再按RBSA公式,计算实际烧伤面积。结论四肢残缺患者与正常人相比,在心理、生理及病理生理等方面有所差别,一旦发生烧伤,无疑会对上述指标产生影响;准确地评估缺肢患者的烧伤面积,既有诊断意义,更有临床治疗和疗效评定意义;用RBSA公式,不仅可计算缺肢成人与儿童的实际烧伤总面积,也可计算各种不同深度创面的实际烧伤面积;RBSA计算公式具备了"新九分法"的特点,可作为"新九分法"的补充。更多还原

【Abstract】 Objective"New rule of nine"has been used to evaluate burn area fro many years and proved to be simple and convenient.However,how to evaluate the burn area of lipomeria patient remains a problem.Such burn area will be underestimated if evaluated by"new rule of nine".The research here provided calculation formula for evaluating the real burn surface area(RBSA) of lipomeria patient.Methods Take parameters of"new rule of nine"as standards,evaluate the percentages of the area of lost parts and burn area with the total body surface area(TBSA) excluding lost parts regarded as 100%.Treat mathematically and find the rule to calculate the supplemental percentage for every 1%burn area evaluated by"new rule of nine"to obtain the calculation formula of RBSA.Results RBSA of lipomeria patient(%TBSA) = estimated percentage of burn area X(1 + percentage of lost parts/100);For children losing lower limbs,the percentage of lower limbs’ area is less than that of adult,so calculate the percentage of lost limbs’ area first according to the"new rule of nine",then calculate the RBSA accordingly.Conclusion Compared to normal person,patients losing limbs are different in aspect of psychology,physiology and pathophysiology.Once burned,the burn event will certainly have an effect on the indices of said aspects.Accurate evaluation of burn area of lipomeria patient has significance of clinical treatment and effect assessment as well as significance of diagnosis.RBSA formula can be used not only for the RBSA calculation of adult and children losing limbs,but for the RBSA calculation of wound surfaces of different depth.RBSA formula has the features of"new rule of nine"and can be its supplement.更多还原
